MAIN DUTIES AND RESPONSIBILITIES OF THE POST: OVERVIEW and SPECIFIC
RESPONSIBILITIES
Clinical Pharmacy Service
(CPS) The CPS will be delivered 5 days a week in the PCN. The
clinical pharmacy service will be primarily patient facing and clinical in
nature focused on improving medicines optimisation and safety working as part
of a multidisciplinary team.
Main elements include:
Supporting the delivery of the
pharmacy and medicines optimisation elements of the network service
specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification.
Undertaking structured medication review and optimisation to tackle over-medication of patients, in
particular elements will include:
Inappropriate use of antibiotics Withdrawing medicines no longer
needed
Structured medication reviews
to support patients
to take their medicines to get the best from them, reducing waste
and promoting self-care.
Dedicated focus on priority
groups as per IIF DES
a. in care homes
b. with complex and problematic polypharmacy, specifically those on 10 or more medications
c. on medicines commonly
associated with medication errors
d. with severe frailty,
13 who are particularly isolated
or housebound or who
have had recent hospital admissions and/or falls
e. using one or more potentially addictive medications from the following groups: opioids;
gabapentinoids; benzodiazepines; and z-drugs.
Management of medicines after
discharge from hospital, reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying
unexplained changes and escalating to GP lead in the practice if out of scope
of practice of the individual.
Work in partnership with secondary care colleagues to proactively manage
patients at high risk of medicine related problems before discharge to
ensure continuity of care.
Review the use of medicines
most commonly associated with unplanned hospital admissions and readmissions through
audit and individualised patient reviews. Put in place changes to reduce the prescribing
of these medicines to high risk patient groups.
Ensure that patients are referred to the appropriate healthcare professional for the
appropriate period of time e.g. acute conditions, minor ailments, long term conditions.
Undertake and complete the Primary care Pharmacy Education Programme if required
Support the delivery of the Medicines Safety agenda and medicines related
QoF quality improvements
Collaborate with pharmacy colleagues across the different sectors to ensure medicines optimisation across the
interface.
Provide a leadership role in supporting further integration of general practice
with the wide healthcare
teams to help improve patient outcomes
Support the PCN Pharmacy
Technician with their
work planning, work load.
Provide pharmaceutical education and training to other healthcare staff in the
PCN
To follow all Health
and Safety standards in compliance with practice
policies and procedures. This includes report
all accidents, incidents and near misses
via local NHS Surrey Heartlands
procedures.
Carry out the following
in all areas of work to:
Develop, review and follow all standard operating
procedures (SOPs) accurately.
Maintain accurate manual
and computer records. Housekeeping duties
Ensure professional and legal standards
are adhered to
To participate in specified interface project/audits,
according to need that supports medicines optimisation activities. The definition of these projects
will link to issues
arising at PCN level and actions arising out of the medicines safety QoF QI
pertaining to interface matters. Supervised support will be given for these projects.
All post holders will:
Will receive a minimum of one supervision session per month by a
senior clinical pharmacist (this does not
need to be part of the PCN and could be part of a local wider network or another PCN).
Will receive a minimum of one supervision session every 3 months by a GP Clinical supervisor. Each clinical
pharmacist will be assigned a GP clinical supervisor for support and
development.
To maintain registration, as a pharmacist, with the General
Pharmaceutical Council, and to
act in accordance with the code of ethics.
The post holder agrees
to participate in Continuing Professional Development (CPD)
including further training and learning in order to develop the knowledge,
understanding, skills, attitudes and values to become competent in the duties
described. Willing to enrol on CPPE pathway
training programme for primary care.
To undertake peer review
and competency based
assessment to ensure
required standards of service are maintained.
Be a member and attend meetings
with of the North West Surrey Clinical Pharmacy Network quarterly on a Wednesday
/ Thursday.

Your hours are based on a whole
time week of 37.5 hours.
Hours are calculated excluding meal breaks and are to be
worked in accordance with the needs of the service and as agreed with your manager.
The way in which the contracted hours are worked may change in accordance with the needs of the
service, following consultation.
