Job summary

We are a small, family-focused GP practice in Islington looking for a skilled Nurse Associate to join our friendly team.

Youll be confident in cervical screening (smears) and childhood immunisations, providing high-quality care to patients of all ages in a supportive environment.

Main duties of the job

Carry out smear tests in line with national guidelines.

Deliver childhood immunisations and vaccines.

Support chronic disease management and patient care.

Maintain accurate clinical records and contribute to quality improvement.

Job description

Job responsibilities

To provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Demonstrates safe clinical decision-making and expert care, inclusive of assessment and diagnostic skills, for patients within the surgery.

Work collaboratively within both the clinical and administrative teams.

Key Responsibilities.

Assess, plan, implement and evaluate treatment / interventions and care for patients.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide nursing treatments to patients in participation with GPs or independently to agreed protocols.

Provide general and specific health screening to the practice patients (within agreed protocols with referral to GPs as necessary).

Advise patients on general health care (with referral to GPs as necessary).

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Meet the needs of patients presenting for opportunistic wound care.

Undertake the collection of pathological specimens including intravenous blood samples and swabs. Perform any investigatory procedures requested by GPs if competent to do so.

Communication.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate with and support patients who are receiving bad news.

Communicate effectively with patients and carer, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural backgrounds and preferred ways of communicating.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication within the practice environment and with external stakeholders.

Act as an advocate for patients and colleagues.

Ensure awareness of sources of support and guidance (e.g. cervical screening leaflets) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Recognise and work within own competence and professional code of conduct as regulated by the NMC.

Produce accurate, contemporaneous and complete records or patient consultation, consistent with legislation, policies and procedures.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Assist in the updating of practice protocols, standard operating procedures and policies.

In participation with the nurse manager, ensure the maintenance of equipment and stock levels meet patient requirements.

General Duties.

E-mail read them daily and take necessary action on any message received.

Tasks and notifications read daily and take necessary action on any tasks or briefings.

GPTeamNet check daily, acknowledge items as necessary. Update the required HR checks such as training courses attended, indemnity, etc.

Standard worksheets follow and when necessary update / create worksheets as appropriate.

Receive and make calls as required. Divert calls and take messages as appropriate, create a task for all messages to other clinicians and administrative staff. Do not use paper. Log all dialogue with patients in Systmone.

Check the room rota daily for any changes. Report any issues / errors to the GP Partners or Practice Manager.

Attend meetings as required.

Ensure that your name plate is put in place and removed for each clinical session.

Professional Development / Revalidation.

Maintain continued professional development in accordance with the NMC revalidation guidelines.

Ensure NMC registration is maintained and indemnity is accurate and up to date.
