Job summary
Undertake a range of nursing assessments and provide appropriate
care/treatment in conjunction with a multidisciplinary team according to practice policy and
protocols, on a full time basis. Hours/days worked are negotiable.
Chronic disease management is an essential part of the role.
Knowledge of the GMS contract, Quality and Outcomes Framework (QOF),
Impact and Investment Fund (IIF) and Healthy Fylde Coast Contract (HFCC)
Main duties of the job
Assist a
Multidisciplinary Team to deliver effective implementation of QOF, IIF and HFCC
Contribute
towards the development of policies/protocols within the practice
Assist the practice
team in implementing new initiatives in the delivery of chronic disease
management
Promote the
continuing professional development of yourself
You are responsible to
limiting your own actions to those that you feel competent to undertake.
Particiapte
in an annual appraisal
Prepare
audits/reports as required

We are a rural practice with 9700+ patients.
We are a 5 GP partnership, 2 salaried GPs, 2 ANPs and a Trainee ACP, 2 Practice Nurses and 2 HCAs.
