Community nurse in primary care

We have an opportunity for a new community care nurse for our large group of GP practices in north southwark

Nexus is a group of 9 practices who merged from several GP partnerships in 2016

We serve patients across north southwark in an exciting and diverse area in central london with great transport links.

Nexus has an established Nursing team of Practice Nurses, community nurses, Nurse associates and Health care assistants who all work as one team in a supportive enviroment. We have standardised induction, training and pay scheme

We are committed to developing staff and also work with a wide range of other practitioners and support patients holistically as a multi disciplinary team

The role involves managing the routine reviews for long term conditions of housebound patients as well as follow ups and required. The nurse will be supported by other community Nurses as well as the wider medical and practice nurse team

The community nurses are also involved in the delivery of seasonal vaccines for our housebound patients

About us

Nexus was formed in 2016 with the merging of a number of established local practices in north Southwark.We look after a practice population of 76,000 patients spread across our 9 sites. The overall aim of Nexus Health Group is to provide a clinically excellent service to our patients, recognising the needs of our local population and to provide a learning and developmental environment for our staff. Our clinical approach is based on a foundation of clinical excellence, clinical leadership and clinical governance. We are a well recognised training organisation.

Our 9 sites across north Southwark are geographically close and well served by local transport hubs at London Bridge, Elephant & Castle and Canada Water.We share a single patient list but our patients are attached to a specific site and are looked after by local multidisciplinary teams which include GPs, Nurses, Physicians Associates, Paramedics, ANPs and Pharmacists. These site based teams are supported by Managers and administration staff, as well as centralised HR, IT, Clinical Administration and the Senior Management Team.

All our sites function as traditional practices with a Site Manager and GP partners working in the practice. All sites are involved in GP training and increasingly training the wider AHP team such as nurses and PAs. Nexus works closely with the local PCN with 2 partners as Clinical Directors.

Essential

  • Minimum of 2 years post graduate experience in a community or primary care setting
  • Working within a multi-disciplinary team
  • Experience of managing a case load, implementing care pathways, protocols and clinical guidelines.
  • Experience of management of complex long-term conditions and can demonstrate the importance of chronic disease management and rehabilitation in the community.

Desirable

  • Worked as community or district nurse
  • Minimum of 2 years post graduate experience

  • Registered Nurse
  • Current NMC Qualification
  • Post registration courses in chronic disease management

Desirable

  • Independent non-medical prescriber (V300)
  • Primary care experience
