Main duties of the job
Access Hub Coordinator -Working in our central access team, our AHCs support patients to access services in the most effective way possible. This role requires people with great communication skills, empathy and a willingness to help our patients and staff. Full job description available
Patient Service Advisor -Working within our GP surgeries, our PSAs patients and visitors to access services, and respond to queries at the front desk (face to face). Full job description available
Referral Management Administrator -To ensure that the flow of referrals and reports coming in and out of Nexus is maintained to the required standard. Full job description available
Bank workers- This will be as and when there is availability. The roles could be based in our GP surgeries, or in our back office teams
About us
Nexus was formed in 2016 with the merging of a number of established local practices in north Southwark.We look after a practice population of 76,000 patients spread across our 9 sites. The overall aim of Nexus Health Group is to provide a clinically excellent service to our patients, recognising the needs of our local population and to provide a learning and developmental environment for our staff. Our clinical approach is based on a foundation of clinical excellence, clinical leadership and clinical governance. We are a well recognised training organisation.
Our 9 sites across north Southwark are geographically close and well served by local transport hubs at London Bridge, Elephant & Castle and Canada Water.We share a single patient list but our patients are attached to a specific site and are looked after by local multidisciplinary teams which include GPs, Nurses, Physicians Associates, Paramedics, ANPs and Pharmacists. These site based teams are supported by Managers and administration staff, as well as centralised HR, IT, Clinical Administration and the Senior Management Team.
All our sites function as traditional practices with a Site Manager and GP partners working in the practice. All sites are involved in GP training and increasingly training the wider AHP team such as nurses and PAs. Nexus works closely with the local PCN with 2 partners as Clinical Directors.
