Job summary
Highfield Surgery (25 Severn Street, Leicester, LE2 0NN) is
currently looking for a part time practice nurse to join our
dynamic and friendly team.
We offer a competitive rate, 27 days annual leave (pro rata)
and NHS pension scheme. Youll be working with a GP surgery led by 2 extremely
supportive GP Partners, focusing on each individual strengths and career
development, a friendly and dynamic hardworking nursing team, and a flexible
and highly trained administrative and reception team, serving for up to 4,800
patients. We are rated CQC good and perform highly in QOF. We are actively
working with local organisations, with one of the GP partners being a clinical
lead of Across Leicester PCN, LMC as well as the strategic clinical lead of the
newly formed ICS.

Main duties of the job
The following are the core responsibilities of the lead practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being
To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
To diagnose and manage acute and chronic conditions
To review patient medications to enhance compliance
To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosagesn.
To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
To maintain accurate clinical records in conjunction with extant legislation
To ensure read codes are used effectively by all members of the nursing team
To ensure the appropriate maintenance and use of chronic disease registers
