Job summary

We are seeking to recruit an experienced Practice Nurse to join our dynamic, friendly nursing team at Paxton Medical Group.

We are a growing practice serving a population of over 20,000 patients across three sites; Claughton, Fender Way and Townfield.

You will be working in an experienced, well established nursing team within a training practice.

Main duties of the job

As an integral member of our clinical team, you will provide a wide range of nursing services to our diverse patient population. You will work autonomously while being fully supported by our GP's, Nurses, HCA's, and administrative staff.

Your responsibilities will include

  • Chronic disease management (e.g., asthma, COPD, diabetes, hypertension)
  • Supporting long term condition reviews and health promotion initiatives
  • Contributing to QOF, enhanced services, and practice wide clinical targets
  • Providing evidence based, patient centred care in line with current guidelines

We are looking for someone who brings both experience and a positive, proactive approach. You should have:

  • Previous experience working as a Practice Nurse in primary care
  • Strong clinical assessment and decision making skills
  • Up to date training in key areas such as cytology, immunisations, and chronic disease management
  • Excellent communication and organisational abilities
  • A commitment to continuous professional development

  • Knowledge of infection prevention and control, medicines management, and cold chain processes.
  • Ability to deliver health promotion and self management education.

Desirable

  • Experience contributing to QOF, IIF, or other performance frameworks.
  • Experience supervising or mentoring junior staff, HCAs, or students.
  • Training in travel health or vaccination programmes.
  • Familiarity with digital clinical systems (e.g., EMIS)

  • Registered Nurse (NMC) with experience in primary or community care settings.
  • Post registration training relevant to primary care (e.g., cervical cytology, immunisations, wound care, phlebotomy).
  • Competency in chronic disease management (e.g., Asthma, COPD, Diabetes, Hypertension).

Desirable

  • Independent or supplementary prescribing qualification
  • Specialist training in diabetes or respiratory care
