Job summary
We are seeking to enhance our innovative and dynamic primary care network (PCN). Our PCN is a group of 7 GP practices, working together and developing a new team of shared staff, to complement the practice teams. As a network, our focus is on getting the best out of people, creating a sense of belonging and working within/across teams. We are looking to recruit a candidate who is passionate about working with and supporting our patients and local community.

Main duties of the job
Providing clinical and administrative onsite support for PCN and practice clinics. Currently PCN clinics include Women's health, Chronic pain, Diabetes and Mental health support groups (daytime and adhoc evening sessions). Learning disability health checks- contacting patients and supporting with health checks
Contacting patients to attend various clinics and taking a proactive approach in reaching out to non responders, DNAs and high frequency attenders to services and promoting wider service offers to support patients.
Early cancer diagnosis- being a cancer champion for the practices- increasing cancer screening uptake (bowel, breast and cervical).
Dipping urine, taking blood pressure, carrying out ECGs & phlebotomy where necessary. Phlebotomy would be an essential skill for this role.
Vaccinations and immunisations support- contacting hard to engage patients both childhood and adult immunisations
Cardiovascular prevention (CVD)- supporting BP checks, hypertension case finding
Work closely with other teams in the network to support with seamless flow of care for patients. This includes attending multi disciplinary teams meeting as required
Providing emergency cover for reception at practices
Explaining and promoting healthy lifestyle choices both within practices and community drop in sessions for patients
The PCN the role of the GPA will evolve over time. We are happy to train the right candidate.
