Job summary

Heeley Plus Primary Care Network are a group of 7 practices in Sheffield, serving a population of 40,000 patients. We are an active network who are currently actively engaged in work with our local health and social care organisations to support and develop new care models for our patients and community. We are looking for a committed, flexible and innovative digital and transformation operations assistant to support us with administrative functions, network projects and support with data management.

Main duties of the job

The PCN Operations Assistant will be responsible for undertaking a wide range of day to day administrative and project support to the Primary Care Network and team and to support to deliver ongoing improvement to services.

The PCN Operations Assistant will take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.

The PCN Operations Assistant will assist the PCN Manager with their relationships and engagement with the PCN practices and other stakeholders across the local neighbourhood to ensure a collaborative approach to service development and delivery.

The PCN Operations Assistant will assist the PCN Manager in ensuring the dissemination of information across the member practices and tasked with supporting the administration of any shared learning events and projects.

The PCN Operations Assistant will support the PCN Manager in the delivery of training, development and support to PCN staff.

The PCN Operations Assistant will support in managing PCN employments and induction of new members of staff. They will also support in line managing some members of staff.

The PCN Operations Assistant will support the PCN with digital developments, data management and reporting and provide onsite and administrative support for projects in the PCN. Some projects and meetings will require support during evening hours.

Job responsibilities

The following are the core responsibilities of the PCN Digital and Transformation Operations Assistant. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

a. To maintain and enhance collaborative relationships across the PCN, as guided by the PCN Manager and Clinical Director

b. Being the first point of contact for PCN roles to support with operational queries and support to do their day to day roles. To manage records for staff in the PCN and ensure reporting on staffing is up to date.

c. Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges

d. Manage staff availability for services and administer rotas for projects in the PCN

e. To undertake calendar management and effectively organise meetings and events.

f. To support with minute taking at meetings and ensuring these are communicated to the various teams in a timely manner

g. To maintain, improve and develop administration systems involving proficient use of MS Outlook, Teams, Word, Excel, PowerPoint ability to touch type.

h. To support with maintenance of PCN website, creating posters and flyers for communication about services and managing social media

i. To monitor performance of the PCN through contract performance databases to help the PCN successfully achieve the requirements of the PCN Network DES.

j. To process documentation, file and store records, project plans and data as required

k. To provide general administrative support to the PCN Manager in their role

l. To support the Clinical Directors with general administrative tasks as requested

m. To support in the delivery of PCN Network DES and other service requirements and targets on behalf of the PCN

n. To undertake all mandatory training and induction programmes

o. To contribute to and embrace the spectrum of clinical governance

p. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

q. To contribute to public health campaigns (e.g., flu clinics, health cafes, drop ins) through advice or direct care

r. To maintain a clean, tidy, effective working area at all times
