Job summary

To provide a comprehensive pharmacy service to the patients of the Greenwell Primary Care Network (PCN).

The post holder will work as part of a multi-disciplinary team.The post holder will take responsibility for areas of medication safety within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide clinical leadership on patient centred medicines optimisation and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.

The post holder will ideally be a prescriber or will be completing training to become a prescriber and will work alongside the general practice teams.

Main duties of the job

Patient facing Long-term condition reviews - Take responsibility for the care management of patients with chronic diseases where medicine optimisation is required. E.g., COPD, asthma.

Patient facing clinical medication reviews - Undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care.

Management of repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.

Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, community pharmacists and anticoagulation.

Complete medication safety audits- MHRA and local.

Improvement of processes related to prescribing within practices

About us

Greenwell PCN covers 7 GP practices in Greenford and Hanwell.

Our patient population is approximately 40,000.

We have practice based Clinical Pharmacists at each practice who works to meet the CP needs of the practice and the overall PCN targets.

Our CPs work together in our hub regularly so benefit from working as a team, supporting each other whilst still having strong relationships with their assigned practice.

We support personal development and training.

KEY RESPONSIBILITIES:

Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, community pharmacists and anticoagulation.

3) Knowledge, skills and experience required

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

Minimum of 1 years experience as a pharmacist.

May hold or be working towards an independent prescribing qualification.

Have experience and an awareness of common acute and long term conditions that are likely to be seen in general practice.

Recognises priorities when problem solving and identifies deviations from normal pattern and is able to refer as appropriate.

Able to follow legal, ethical, professional and organisational policies and procedures and codes of conduct.

Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

4) Leadership and management

The post holder is managed and will supervise him/herself by making decisions regarding meeting targets independently, producing reports highlighting performance information.
