Job responsibilities
KEY RESPONSIBILITIES:
Patient facing Long-term condition reviews - Take responsibility for the care management of patients with chronic diseases where medicine optimisation is required. E.g., COPD, asthma.
Patient facing clinical medication reviews - Undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care.
Management of repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
To have a leadership role in supporting further integration of general practice with the wider health care teams (community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. The role has the potential to significantly improve the quality of care and safety for patients.
Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.
Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, community pharmacists and anticoagulation.
Complete medication safety audits- MHRA and local.
Improvement of processes related to prescribing within practices
3) Knowledge, skills and experience required
Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
Minimum of 1 years experience as a pharmacist.
May hold or be working towards an independent prescribing qualification.
Have experience and an awareness of common acute and long term conditions that are likely to be seen in general practice.
Recognises priorities when problem solving and identifies deviations from normal pattern and is able to refer as appropriate.
Able to follow legal, ethical, professional and organisational policies and procedures and codes of conduct.
Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

The post holder is managed and will supervise him/herself by making decisions regarding meeting targets independently, producing reports highlighting performance information.

6) Confidentiality:
- As per both Government legislation and Practice policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to, reporting any infringements to the PCN Clinical Director or Manager.
7) Health & Safety:
- The post-holder will assist Connexus PCN H&S lead in promoting and maintaining their own and others health, safety and security as defined in the Connexus PCN Health & Safety Policy and related Risk Assessments.

9) Professional Development:
- Maintain continued education by attendance of courses and study days as deemed useful or necessary for professional development and Connexus PCN needs.
- Attend annual mandatory courses.

Use of SystmOne.
Commitment to the use of IT, data entry, read coding and targets.
Extraction of data and use of spreadsheets to analyse and present information to determine trends or highlight potential problem, as required by the Connexus PCN.
