Job summary
Working as part of HASP PCN, the Care Coordinators
will take the lead on specific initiatives within the PCN, namely enhanced
health in care homes, and the extended access initiatives. The roles are highly dynamic however and
duties may change in response to new initiatives or changing priorities within
the PCN.
The successful
candidates will act as first contacts for their specified initiatives and will
lead on the implementation of the specific goals attached to each
initiative. Key aspects of the roles
include communicating with a wide range of organisations and/or individuals,
planning service delivery and ensuring that the services provided by the PCN
run efficiently and effectively.
Main duties of the job
Proactively
identify and work with a cohort of people to support their personalised care
requirements, using the available decision support aids
Bring
together a persons identified care and support needs and explore their options
to meet these into a single personalised care and support plan, in line with
person-centred service plan (PCSP) best practice
Help people
to manage their need, answering their queries and supporting them to make
appointments
Ensure that
people have good quality information to help them make choices about their care
Support
people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing, including
through use of the patient activation measure
Assist people
to access self-management education courses, peer support or interventions that
support them in their health and wellbeing
Provide
coordination and navigation for people and their carers across health and care
services, alongside working closely with social prescribing link workers,
health and wellbeing coaches and other primary care roles
Support the coordination and delivery of MDTs within PCNs

effectively and continually evaluated. The postholder will work with the
Governing Board, Clinical Director, Strategic Manager and PCN Member practices
and other bodies as required for the delivery of specific objectives (depending
on the initiative itself) to improve the quality and experience of the services
we provide to our patients.
