Main duties of the job
The Lead Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure and supporting the GP with areas such as Safeguarding. This role will also be intergral to the service development and support of other clinical roles within service.

We are looking for a Part time Lead Practice Nurse - hours negotiable within 20 to 25 hours per week.
The successful candidate will work as part of the primary healthcare team providing Personal Medical Services to the patients of practice

Job Responsibilities:
To take a lead on supporting nursing activity within the practice
Support the achievement of the Quality Outcomes Framework (QOF) and Local Enhanced Services (LES)
Providing assessment, screening and treatment services and health education advice
Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.
Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to general practitioners as necessary.
Advise patients on general health care and minor ailments with referral to GPs as necessary.
Routine tasks may include, but are not limited to:
Chronic disease management and review (including diabetes, hypertension, CKD, dementia, asthma, COPD, cancer, palliative , mental health etc)
Disease prevention and screening (cervical smears, smoking cessation, immunisations and vaccines, obesity, alcohol, etc.

Ensure the maintenance of equipment and stock levels relating to patient care.
Ensure that the practice is meeting it requirements for Infection Control
Pathological specimens and investigatory procedures
Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs
Administrative and professional responsibilities
Participate in the administrative and professional responsibilities of the Practice team.
Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
Attend and participate in Practice meetings as required.
Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.
Research projects
Co-operate and participate as required in any research projects within the Practice.
Student/staff education and training
Participate in the education and training of others of all disciplines and the induction of all members of Practice staff with special responsibility for nursing staff.
Disseminate learning and share best practice with team members to keep the practice informed about current and upcoming developments, such as NICE guidance, Local Enhanced Services (LES), Quality and Outcomes Framework (QOF) updates, and relevant training opportunities.
