Job summary
Hammersmith and Fulham Central PCN is seeking a motivated and experienced Paramedic to join our team under the ARRS scheme. The successful candidate will play an important part in providing high quality patient centered care to our housebound patient population across five practices.
Main duties of the job
Working alongside GP's, nurses and the wider primary care team the successful candidate will support continuity of care to our housebound patients through the provision of an acute home visit service and proactive management of long term conditions.
They will also provide clinical care within practices including same day triage clinics, post-discharge reviews and minor illness management, within a team consisting of GP's, nurses, pharmacists and HCA's.

The post holder will:
- work as part of a multi-disciplinary team within the PCN.
- assess and triage patients and as appropriate, provide definitive treatment following policy, patient group directives, NICE, local clinical guidelines and local care pathways or make necessary referrals to other members of the primary care team.
- advise patients on general healthcare and promote self- management where appropriate.
- be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.
- be able to perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice.
- support the delivery of 'anticipatory care plans' and lead certain services e.g. monitoring blood pressure and diabetes risk of elderly patients.
- provide an alternative model to urgent and same day GP home visits for the network.
- communicate at all levels across across organisations ensuring that an effective patient-centered service is delivered.
- communicate proactively and effectively with all colleagues across the team, attending and contributing to meetings as required.
- maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the practice.
- communicate effectively with patients and where appropriate family members and their carers, when applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices.

- Completed a two year 'consolidation of learning' period when a newly qualified paramedic.
- Completed a further three years experience as a band 6 (or equivalent) paramedic.
Desirable
- Previous experience of working in a primary care setting.
- Knowledge of SystmOne clinical system.
Qualifications
Essential
- BSc in Paramedic Science or equivalent qualification (Level 6).
- Full HCPC registration as a Paramedic.
- First Contact Practitioner Post-Graduate Certificate or Portfolio Qualification Equivalent

- Valid Full UK Driving Licence
