Summary
Central
and South Primary Care Network is seeking two highly skilled and motivated Advanced
Nurse Practitioners (ANP) or Advanced Clinical Practitioners (ACP)
to join our expanding multidisciplinary team.
These
are ARRS-funded roles working across Park House Medical Centre
and Tarbock Medical Centre, offering the opportunity to deliver
high-quality, autonomous clinical care within supportive and well-established
general practice environments.
We
are looking for practitioners with a broad scope of practice and an
appropriate personal formulary, confident in managing undifferentiated
presentations and working independently as autonomous clinicians.
The
successful candidates will be employed by Roby Medical Centre on behalf
of Central and South PCN.

Main duties of the job
About
the Role
You
will work autonomously to assess, diagnose, and manage a range of patients,
including those with complex or long-term conditions. You will demonstrate
sound clinical judgement, evidence-based practice, and a patient-centred
approach in all aspects of care.
You
will contribute to the PCNs wider clinical network, supporting safe,
effective, and sustainable primary care delivery. Both roles are designed for
experienced ANPs or ACPs who meet the ARRS eligibility criteria and are ready
to work independently within a supportive, structured environment.

- Clinical Expertise
- Independently assess, diagnose, plan, and implement care for patients across the lifespan, including those with chronic disease management needs.
- Provide evidence-based clinical interventions and prescribe medication as appropriate under independent prescribing rights and in line with current guidance Pan Mersey, and NICE guidance.
- Manage patients presenting with undifferentiated and undiagnosed problems, ensuring care aligns with NHS England (NHSE), Care Quality Commission (CQC), and Royal College of Nursing (RCN) standards.
- Autonomous Practice
- Work as an autonomous practitioner, taking accountability for clinical decision-making and patient outcomes.
- Undertake clinical risk assessment, ensuring care delivery adheres to national guidance, including NICE and NHSE protocols.
- Chronic Disease Management
- Deliver high-quality care for patients with chronic conditions such as diabetes, asthma, and hypertension.
- Promote patient self-management and provide education to improve long-term health outcomes.
- Quality and Governance
- Ensure all practice aligns with CQC standards and contribute to audits, quality improvement initiatives, and governance processes within the practice.
