Job summary

We are looking for a skilled and motivated Advanced Clinical Practitioner (ACP) to join our friendly and forward-thinking team at Chilcote Practice. Main duties of the job

  • Deliver autonomous, high-quality care within your professional scope and code of conduct.
  • Assess, diagnose, and manage patients with acute and chronic conditions.
  • Prescribe and manage medications and interventions in line with current guidance.
  • Lead health promotion and wellbeing initiatives.
  • Collaborate with patients, families, carers, and multidisciplinary teams to plan and deliver care.
  • Respond to clinical emergencies and mental health presentations.
  • Maintain accurate clinical records using SNOMED CT coding.
  • Support vaccination programmes and contribute to QOF and other practice targets.
  • Liaise with external agencies to ensure holistic patient support.
  • Uphold safeguarding responsibilities and current legislation.

    Job description

    Job responsibilities

    a. Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice

    b. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

    c. Act on professional judgement about seeking help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change

    • - Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
    • - Proof of relevant professional and educational qualifications, coupled with details of any training.
    • - Registration with their professional governing body.
    • - Qualified independent prescriber
