Main duties of the job
Practice Nurse Key Responsibilities
As a vital member of the primary care team, the Practice Nurse will deliver high-quality, patient-centred care across a range of clinical areas. Responsibilities include:
Clinical Duties- Chronic Disease Management: Conduct reviews and provide care plans for patients with diabetes, asthma, COPD, hypertension, and cardiovascular disease.
- Health Screening & Promotion: Support NHS Health Checks, smoking cessation, weight management, and lifestyle advice.
- Cervical Screening: Perform smear tests and provide education and reassurance.
- Childhood & Travel Immunisations: Administer routine and travel vaccinations in line with national schedules.
- Wound Care: Manage minor injuries, dressings, and removal of sutures.
- Diagnostic Testing: Carry out ECGs, blood pressure monitoring, spirometry, venepuncture, and urinalysis.
- Sexual & Reproductive Health: Offer contraception advice, STI testing, and assist with coil fittings.
- Minor Procedures Support: Assist GPs during minor surgeries and implant insertions/removals.
- Maintain accurate patient records and documentation.
- Ensure compliance with infection control and health & safety protocols.
- Monitor and manage clinical supplies and cold chain procedures.
- Participate in audits, significant event reviews, and quality improvement initiatives.
- Work within NMC guidelines and practice protocols.
- Engage in ongoing training and contribute to team learning.

Our practice is a 10 partner organisation providing general medical services to a patient list of approximately 15,600 patients in Torbay. The organisation formed in 1986 and is based over 2 sites, Dewerstone at Hampton Avenue and Chatto at Chatto Road.
