Job summary

An exciting opportunity has arisen for an experienced, motivated, Practice Nurse, with an interest in developing Diabetes, to join our dynamic and supportive team at Cardigan Health Centre.

You will be working within a supportive, friendly team across nurses, doctors, reception and admin teams.

We have a Practice population of over 10,300 patients and we work collaboratively with MIU, physiotherapy and community nurses, who are all based within the Integrated Care Centre.

Main duties of the job

The role will primarily focus on Diabetes, but it will also include general Practice Nurse and treatment room duties. You will be responsible for ensuring the delivery of safe and effective nursing care to the whole Practice population, clinically delivering evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. You will work collaboratively with the whole General Practice team to meet the needs of patients, supporting the delivery of procedures.

Experience in Diabetes care and monitoring and in a General Practice environment is desirable, but full training will be given to the right candidate. You will be supported by the GP lead for diabetes and the community diabetes nurses.

Occasional overtime will be expected, as well as a once per annum commitment to weekend working for the purpose of flu clinics.

Main Responsibilities

The Practice Nurse will work as part of the primary healthcare team, providing personal medical services to the patients of the Practice.

Job Responsibilities

  • Help the Practice develop and deliver specialist primary nurse led services including chronic disease management, specifically Diabetes.
  • Initiate the role of diagnosis for patients suspected to have a chronic disease, referring to other clinicians as appropriate
  • Manage programs of care for patients with chronic diseases by planning, providing, and evaluating care under agreed guidelines and patient group directions.
  • Maintain disease registers in liaison with administrative staff.
  • Assist in and perform routine tasks related to patient care as directed by the senior nursing staff and GPs
  • Wound and ulcer care
  • Removal of sutures
  • New patient medicals / urinalysis
  • ECGs
  • Asthma / peak flow readings / Spirometry
  • Venepuncture
  • Hypertension management
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
