Job summary

We are looking to recruit an experienced and enthusiastic prescribing pharmacist. This role will be practice based, but you will work closely alongside the rest of the pharmacy team, which consists of a practice based pharmacy technician and two PCN clinical pharmacists.

The successful applicant will play a lead role in medicines management, optimising prescribing and improving quality. The post holder will undertake medication reviews to proactively manage patients with complex polypharmacy and will implement prescribing decisions and request appropriate monitoring investigations.

The post holder will work closely to support the wider practice teams to support safe and effective prescribing.

We are looking for someone to work between 26 and 37.5 hours. Please make it clear in your application the hours that you are looking for.

Main duties of the job

We are seeking a prescribing pharmacist to work with our clinicians and patients to carry out medication reviews, practice as an independent prescriber working within the scope of their practice and carry out face to face and telephone consultations with patients as required. The ideal candidate will be able to encourage cost effective prescribing within the practice, ensure medications are prescribed and monitored safely and assist with QOF and IIF targets, and other patient care incentives.

About us

Overdale Medical Practice is a well established surgery covering a number of villages on the Nottingham / Derby border. We have over 11,500 patients and the successful applicant will be supported by a multi-disciplinary team of nine GPs, three advanced nurse practitioners, two long term condition nurses and three other practice nurses. Through our PCN we also have a pharmacy team, first contact physio, social prescribing team and mental health practitioner. We benefit from a highly experienced administrative team and reception team. We aim to provide the highest quality care to our practice population. We receive excellent patient feedback, which has us ranked within the top 4% of practices in the country.

Job responsibilities

Carry out structured medication reviews on patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests

Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

Reconcile medicines from clinic letters, with support from the pharmacy technician

Support long term condition reviews

Care home ward rounds to carry out medication reviews and deprescribe where appropriate

Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues

Interface with community and hospital pharmacy colleagues

Manage patients and health care professionals medicine queries

Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles

Contribute to service improvement through QOF and enhanced services

Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety

Work with primary care professional and patients to implement NICE and other evidence based guidelines

Contribute to multi-morbidity reviews and multidisciplinary reviews

Provide medicine information and training to Practice healthcare professionals and admin staff

Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks)

Prescribing advice to prescribers in practice e.g. temporary non availability of drugs

Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines
