PCN Clinical Pharmacist in General Practice

Job summary

We are seeking an enthusiastic and motivated Clinical Pharmacist to join our Primary Care Network team.

This is an exciting opportunity to work in a patient-facing role within general practice, supporting medicines optimisation, improving patient outcomes, and contributing to high-quality care.

You will work closely with GPs, nurses, and the wider multidisciplinary team, delivering clinical services and supporting safe and effective prescribing.

Main duties of the job

  • Undertake structured medication reviews and manage polypharmacy
  • Support patients with long-term conditions
  • Provide expert medicines advice to clinicians and patients
  • Reconcile medicines following hospital discharge
  • Contribute to medicines safety and clinical governance
  • Support repeat prescribing systems
  • Deliver education and training to practice staff

  • Lead on medicines optimisation, safety, and antimicrobial stewardship
  • Deliver patient-facing clinics and medication reviews
  • Be supported to develop into an Independent Prescriber

Desirable

  • Postgraduate diploma or equivalent experience
  • Independent Prescriber qualification

  • Minimum 3 years post qualification experience
  • Strong clinical and therapeutic knowledge
  • Understanding of general practice and primary care prescribing
  • Excellent communication and interpersonal skills
  • Ability to manage medicines optimisation issues
  • Good IT skills
  • Ability to work autonomously and within a team

Desirable

  • Experience in primary care
  • Familiarity with SystemOne
  • Motivational interviewing skills
  • Ability to train and support others
