Job summary
We are seeking an enthusiastic and motivated Clinical
Pharmacist to join our Primary Care Network team.
This is an exciting opportunity to work in a patient-facing
role within general practice, supporting medicines optimisation, improving
patient outcomes, and contributing to high-quality care.
You will work closely with GPs, nurses, and the wider
multidisciplinary team, delivering clinical services and supporting safe and
effective prescribing.
Main duties of the job
- Undertake structured medication reviews and manage polypharmacy
- Support patients with long-term conditions
- Provide expert medicines advice to clinicians and patients
- Reconcile medicines following hospital discharge
- Contribute to medicines safety and clinical governance
- Support repeat prescribing systems
- Deliver education and training to practice staff

- Good IT skills

- Familiarity with SystemOne
