To provide operational leadership and project management expertise to implement approved service initiatives across our local network. The Service Implementation Manager will work within the Core PCN Team to support the Chief Operating Officer, Clinical Directors and member practices to execute and deliver service improvement projects aligned to local and national priorities. This includes operationalising new service models, managing project timelines and budgets, and ensuring successful adoption of initiatives across all member practices to deliver sustainable, proactive, personalised, coordinated and more integrated health and care.

Main duties of the job

The Service Implementation Manager has operational responsibility for the execution and delivery of PCN projects and services. They are responsible for translating strategic plans into actionable deliverables, monitoring implementation progress, managing project budgets, and ensuring successful project outcomes that meet our PCN objectives and local population needs.

Job responsibilities

Project Management & Delivery

  • Develop and maintain comprehensive project documentation including Project Initiation Documents (PIDs), business cases, risk registers, RAID logs, and project plans using appropriate PM tools
  • Manage project budgets and financial tracking across multiple initiatives, ensuring delivery within approved financial parameters and providing regular budget reports to stakeholders
  • Lead project governance meetings, maintain decision logs, and ensure appropriate escalation of risks and issues through defined governance structures
  • Conduct formal project reviews, lessons learned sessions, and post-implementation reviews to ensure continuous improvement
  • Establish and monitor KPIs and success metrics for each project, conducting benefits realisation assessments against agreed baselines
  • Develop contingency plans and actively manage project dependencies across the portfolio

Service Implementation

  • Execute approved revenue generation strategies by implementing new service models and business development initiatives, ensuring successful operational delivery and alignment with PCN strategic goals
  • Manage the implementation of service improvement projects by developing detailed project plans, coordinating resources across member practices, and ensuring timely delivery of agreed solutions
  • Coordinate with member practices and stakeholders to ensure smooth implementation of initiatives. Manage project communications, resolve operational challenges, and ensure consistent adoption of new processes across the network
  • Work within the PCN Executive Team to operationalise PCN DES requirements and ensure contractual deliverables are met across the Network

Stakeholder Management & Leadership

  • Lead project teams to deliver implementation objectives through clear task assignment, progress monitoring, and issue resolution
  • Support Practice Managers in implementing new systems and processes by providing hands-on guidance, coordinating rollout schedules, and delivering operational training
  • Provide regular implementation updates and progress reports to the Chief Operating Officer, PCN Board, member practices and other stakeholders using data-driven insights and clear metrics
  • Facilitate change adoption across the network through structured change management approaches and stakeholder engagement strategies

Performance & Quality

  • Monitor project portfolio performance against agreed timelines, budgets, and quality standards
  • Implement and maintain project quality assurance processes
  • Track and report on benefits realisation for all completed projects
  • Contribute to the development of the PCN's project management framework and standards

  • Educated to degree level or equivalent experience
  • Proven experience in project implementation and delivery within healthcare or similar complex service environments
  • Strong understanding of primary care operations and practical experience in implementing service changes
  • Demonstrated experience with project management methodologies and successful delivery track record
  • Excellent coordination and organisational skills with ability to manage multiple implementation projects simultaneously
  • Experience in operational delivery and managing cross-functional implementation teams
