Experience
of Assessment and Treatment delivery in long term conditions and active
management.
A high
level of competence with IT including in Patient Management Systems such as
EMIS Web.
Risk
Assessment Experience.
Experience
of writing comprehensive clinical notes.
Ability
to communicate effectively in both orally and in writing.
Ability
to develop and maintain constructive relationships.
Awareness
of the issues surrounding vulnerable groups and the boundaries of professional

The role will involve working across any of our 5 local practices
and travel to complete home visits with patients across the locality and so a
full driving licence and vehicle for work purposes is required.
37.5 hours per week inclusive of one day per week to attend University or
complete online study to complete the Roadmap.
Main duties of the job
This
post requires an Occupational Therapist who is highly specialised and can work
at an advanced level of practice within Leek and Biddulph Primary Care Network
(PCN).
The PCN's occupational therapy
service is separate from, but complementary to, the occupational therapy
services currently available within secondary services community therapy and
social services OT. The remit of the PCN OT service is Early Intervention for
individuals with physical or mental health conditions.
The service provides assessment
and intervention programmes for Pain Management, Fatigue Management, and
Cognitive Impairment. Environmental assessments within the home are completed
as appropriate.
The post holder will demonstrate safe, clinical decision-making
and expert care for patients within the PCN. They will need to be autonomous
and carry their own clinical caseload under the supervision of the FCP OTs.
Experience within community services essential.
Upon completion of the roadmap the salary would increase.

local patient care. We are a
multi-disciplinary team, covering Leek and Biddulph, responsible for the
healthcare of just over 50,000 patients.
We are led by Dr Neil Briscoe our PCN Clinical Director and a supportive
management team.
We are a very
forward thinking and innovative PCN who were the first PCN in North
Staffordshire to convert to a Limited Company.
We utilise to the full, the skills and experience of our team members
which includes Clinical Pharmacists, Pharmacy Technicians, Occupational
Therapists, Physiotherapists, Social Prescribers and Mental Health Team. We
