Job responsibilities
- Assist in and perform routine tasks related to patient care as directed by Senior Nursing staff and GPs such as BP checks, urinalysis, venepuncture
Requesting basic pathology tests e.g.urine culture, swabs and bloods as per agreed Clinical Protocols and referral to Senior Practice Nurses or GPs as appropriate
B12 replacement therapy and other ongoing injectable therapy, monitoring as appropriate.
Maintaining Stock within Nursing and Treatment Rooms with appropriate storage and stock rotation
Womens Health
o Cervical Cytology, Vaginal Swabs and Family Planning
o Support and advise women requesting information relating to family planning needs
o Support and manage health needs of women presenting for cervical cytology consultations
Mens Health including Prostate Monitoring and Management
Wound Management
o Assess and care for patients presenting with uncomplicated wounds.In line with Local Guidelines and working alongside the Integrated Nursing Team with more complex Wound Management issues
Ear Care
o Advise patients re Ear self-care and management of wax
o Provide Routine ear care as appropriate and in line with Practice Guidelines and procedures
o Referral to and Signposting to Ear Care Specialist Services
Working with the PHCT in managing the delivery of National and Local Enhanced Services including Learning Disabilities and Mental Health Reviews
Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
Assist Senior Practice Nurses in the ongoing monitoring and management of Long Term Conditions including Diabetes, Hypertension, and CVD.
Provide holistic assessment and health promotion programmes including lifestyle advice on diet, smoking, alcohol intake and exercise that support patients to adopt strategies that promote patients to live healthy lifestyles, encourage self - care.
Anticoagulant Management
o To include INR Clinics and Housebound Home Visits for INR Monitoring
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Able to recognise and manage anaphylaxis according to current UK Guidelines
Able to perform Cardio pulmonary resuscitation according to current UK Guidelines
Chaperoning and assisting patients where appropriate who are being examined by another clinician
Assists GPs with Minor Operations, Coil Fits and Implants where requested
Immunisation (Adult and Child)
Assess patients for Immunisation
Administering vaccinations and providing guidance in accordance with guidelines and Patient Group Directives (PGD)
Assess and delegate to Health Care Assistants and less experienced Health Care Workers the administration of Vaccinations and B12 injections via Patient Specific Direction (PSD), Identifying problems with appropriate management
Manage the administration of appropriate immunisation autonomously in accordance with the Green Book (DOH 2006) Immunisation against Infectious Diseases.
Working with patients with more complex immunisation issues
Ensure Vaccine stock levels are maintained and ordered as per Practice and Local Guidelines
Professional Responsibilities, Audit and Research
Participate in the maintenance of clinical governance systems within the Practice
Use Audit Tools to evaluate quality of work of self and the team, implementing improvements where required
Deliver care according to NICE, Local and Practice Guidelines
Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.
Training and Personal development
Assess and be responsible for own Learning and Training Needs with ongoing Personal Development in line with Revalidation process for NMC Registration
Develop and maintain a Personal Development Plan and meet requirements for Revalidation
Must hold and maintain membership of good standingwith their Professional Regulatory Body (NMC)
Participate in Mandatory Training
The post holder will participate in any training programme implemented by the Practice as part of this employment and will include:
o Participation in Annual Individual Performance Review /Appraisal which includes taking responsibility for maintaining a record of own personal and/or professional development
o Taking responsibility for own development, learning and performance, demonstrating skills and activities to others who are undertaking similar work
o Training requirements will be monitored and will be in accordance with Practice requirements.
o Personal Development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
o Actively participate in educational programmes for whole Primary Health Care Team, including Practice Nurse, Health Care Assistants, Nursing/Medical Students and GP Registrars: .participating in Training, Mentoring and Assessment of competencies as appropriate
o Disseminate learning and information gained to other team members
Team Working
To assist the Practice Nurse Team Manager with the management of Nursing Services in participation with the Practice Manager and GP Partners ensuring relevant information is disseminated to and from the Practice Nursing and Integrated Nursing Team
Understand own role and scope and work towards further development of the role over time
Work as an effective and responsible Team Member, supporting others and developing new ways of working
Support less experienced team members
Delegate clearly and appropriately within the Nursing Team
Prioritise own workload and time management.
Work with the Nursing Team to create opportunities to improve health care and work innovatively
Management of Risk
Responsible for the management of the cold chain ensuring appropriate storage, rotation and disposal of vaccines and drugs
Manage and assess risk within areas of responsibility, ensuring adequate measures are in place to protect staff and patients
Using personal security systems within the workplace according to Practice guidelines
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
Making effective use of training to update knowledge and skills
Apply infection control measures within guidelines using appropriate procedures, maintaining work areas in a tidy and safe way and free from hazards
Reporting potential risks identified
Adhere to the Child Health Clinical Policy on surveillance of vulnerable children.
Managing Information
Use IT as an aid to management in planning, implementation and monitoring of care, presenting and communicating information
Manage information using the clinical system, Computer Decision Support Software, internet and local databases
