time Pharmacy Technician to join our clinical team, working alongside a Clinical Pharmacist, Pharmacy Technician and Prescription Clerk.

Summary
The role will provide an
essential part of our pharmacy team, working with clinicians and nursing team, and in collaboration with other members of the Practice multidisciplinary teams.
Main duties of the job
Key duties and responsibilities:
Manage the repeat prescription process, resolving queries, and reducing medication wastage.
Contribute to Quality and Outcomes Framework (QOF) targets, NICE implementation, and clinical audits.
Work alongside GPs and clinical pharmacists to safely manage prescribing tasks, allowing doctors to focus on complex cases.
It will involve taking responsibility for processes and procedures for administration of prescribing and the development of Practice policies.
Assist with medication safety audits, ensuring compliance with legal standards, and reviewing medication safety alerts (e.g. MHRA).
It will help to support the development of the pharmacy team and help the Practices to deliver the PCN DES, create capacity and access in the practice and drive efficiency.
It will help the Practice demonstrate improvements in prescribing and safety considerations through a variety of different mechanisms.
Provide medicines expertise to patients and other members of the multi-disciplinary team.
To promote self-care and educate service users to allow them to be empowered to make informed choices about treatment.
To work collaboratively and build relations with community pharmacy, secondary care and intermediate care teams.

Clinical Pharmacy in General Practice is part of an exciting transformation programme to develop a new model of care, addressing our ambition to deliver person-centred, coordinated care across the practice population. GP practice technicians work within a multidisciplinary team of healthcare professionals, based within a GP practice, and support the delivery of safe, effective and efficient systems for medicines optimisation, repeat prescribing, reducing medicine waste and maximising patient outcomes.
GP practice pharmacy technicians work under the supervision of a pharmacist and are registered healthcare professionals that are responsible and accountable for their own accurate and safe practice. As a GP practice pharmacy technician, you will receive ongoing training and development about all aspects of practice based prescribing and effective administration and will support the practice in providing medication reviews and achieving Quality and Outcomes Framework targets.
The Clinical Pharmacy in General Practice model is to develop and utilise the role of pharmacy within primary care so it can proactively help patients stay safe, well and out of hospital as well as supporting efforts to reduce the demands on general practice.
JOB RESPONSIBILITIES
CLINICAL
- To support the daily preparation of repeat prescriptions to ensure high quality, safe and timely repeat prescribing.
- To provide supervision, training, and professional leadership to the practices administrative staff.
- To communicate effectively and professionally with patients on the telephone, electronically or face to face.
- To update patients medical records in an accurate and timely manner.
- To support systems to review medication usage and synchronisation of medication supplies; perform amendments to issue durations of prescriptions to match actual medication use duration; co-ordinate and lead efforts to satisfy medication monitoring requirements; liaise with the administrative team in regard to patient discharge information and clinical letters to perform medication reconciliation; assist GPs and pharmacists with medication reviews.
- To respond to medication queries from patients and staff (include medication related triage) and action as appropriate in accordance with practice protocols.
- To build and improve the working relationships with the local Community Pharmacy teams, secondary care and intermediate care teams.
- To support initiatives for antimicrobial stewardship.
- To help plan, develop and support the introduction of new working processes within the practices to optimise the quality of prescribing.
- To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.
- To support the delivery of shared care protocols.
- Provide support for practices to action drug alerts.
- To provide advice to GPs, staff, and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness.
- To assist in the appropriate monitoring and achievement of the annual Medicine Optimisation targets and to liaise with the Medicines Optimisation Team at Devon ICB.
- To support the achievement of the practices prescribing Quality and Outcomes Framework targets, and the Quality Improvement targets.
- To provide medication review services to patients via clinics in the Practices, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
CLINICAL GOVERNANCE
- To participate in the clinical governance requirements within the Practice.
- To practice within the parameters of Practice policies, protocols and guidelines.
- To participate in quality improvement within the practice as necessary.
- Reflect and assess results of own decisions, outcomes and treatment; identify any shortfalls in delivery of patient services and report to relevant management.
- Ensure they follow Health and Safety legislation and COSHH regulations, reporting any breaches as necessary.
- Awareness of personal responsibilities in maintaining knowledge of PGDs, medicines policy and legislation regarding safe and secure medicines management.
