An exciting opportunity has arisen for a motivated
and compassionate Practice Nurse to join our team.
We are a friendly, well established GP
practice serving a diverse patient population with an approximate list size of
5,500 patients. We are committed to delivering high-quality, patient-centred
care and clinical services within a supportive and collaborative team
environment.
We use SystmOne as our
clinical system for patient records, QOF monitoring and audit processes,
ensuring safe, effective and well-coordinated care.
The surgery offers extended hours to meet the
needs of our local community, including evening and Saturday clinics. We
actively support professional development, encourage ongoing training and value
innovation in practice to continually improve the care we provide.
We are looking for a nurse who is enthusiastic,
patient-focused, and committed to delivering high-quality, evidence-based care.
The
successful applicant will work alongside one other Practice Nurse, supported by
a Healthcare Assistant and a Care Coordinator.
Main duties of the job
- Deliver safe, effective, and compassionate nursing care to patients
- Conduct independent clinics for asthma, COPD, diabetes, hypertension and other long-term conditions
- Work collaboratively with the multidisciplinary team
- Travel advice and Vaccinations
- Contraception advice and reviews
- Wound Care management
- Undertaking home visits where appropriate
- Childhood and routine immunisations
- Maintain accurate clinical documentation
- Contribute to service improvement and quality initiatives
Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
Competent in anaphylaxis and resuscitation techniques
To undertake home visits

The Practice Nurse at Manchester Road Surgery will work as a key member of the clinical team, acting within professional boundaries to provide high-quality care for patients. 2. Main Duties and Responsibilities
