Diabetic Lead Nurse

Job summary

Odiham and Old Basing Health Centre are looking to recruit an experienced Practice Nurse with experience in diabetes management and leading teams to join our clinical team in this new end exciting role.

You will be confident in the leading and managing of diabetes reviews and long term condition monitoring, working alongside the wider clinical team to help improve patient health outcomes. Qualified prescriber would be advantagous or motivated to acheive this - full support would be given.

Main duties of the job

The post holder will lead and support the delivery of diabetes care within the practice, including:

  • Running diabetes clinics and annual reviews
  • Supporting management of patients with Type 1 and Type 2 diabetes
  • Monitoring blood results including HbA1c, renal function and lipid profiles
  • Supporting medication reviews and optimisation of diabetes treatments
  • Providing lifestyle advice and patient education to support self-management
  • Supporting insulin initiation and titration where appropriate
  • Identifying patients requiring escalation or specialist referral
  • Supporting the practice in achieving QOF targets for diabetes
  • Contributing to clinical audits and quality improvement
  • Working collaboratively with the multidisciplinary team including pharmacists and GPs
  • Leading the nursing team
  • Carrting out regular audits
  • Infection control lead

The role may also include broader practice nursing duties depending on experience, such as chronic disease reviews, vaccinations and health promotion.

About us

We currently care for more than 13,000 patients and are a semi-rural practice.

The partnership is made up of 3 GP partners, supported by 7 salaried GPs. The practice and its patients along with the relationships of the clinical and administrative teams are long established and the practice has consistently achieved high levels of QOF targets for several years and is focused on continuous quality improvement.
