Job summary
Central North Leeds PCN are looking for a highly motivated and aspirational clinical pharmacist to join our established team of pharmacists and pharmacy technicians.
This role is critical in supporting the delivery of the PCN Contract DES specifications and will help lead improvements to maximise safe, evidenced-based best practice to improve the quality of patient care.
Applications welcome from pharmacists with patient-facing experience from any sector. Newly qualified pharmacists or pharmacists looking for a new direction are encouraged to apply. An interest in working in general practice is essential. As a PCN clinical pharmacist you will receive fully accredited training from CPPE and will be provided with appropriate supervision from a GP clinical supervisor and from senior pharmacists.

Main duties of the job
- Work within your clinical competencies with a high level of support and supervision from the PCN Clinical Pharmacists.
- The role will be structured to provide person-centred expertise in clinical medicines management, medicine optimisation, safer prescribing and quality improvement, whilst contributing to the PCN DES contract, quality and outcomes framework and local enhanced services.
- Undertake a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care
- Perform medication reviews
- You will require flexibility to develop in line with the national service specification in the GP contract and NHS Long Term Plan and in respond to the evolving needs of PCNs
- You will be a forward-thinking, flexible, innovative and enthusiastic about working collaboratively across the team and keen to deliver the ethos of the network
- Deal with management of medicines on transfer of care: discharge letters, treatment advice notes and medicines reconciliation
Support to develop, includes enrolment on the Primary Care Pharmacy Education Pathway from CPPE and becoming an independent prescriber. You will have line-management from the Senior Clinical Pharmacist, and a GP clinical supervisor. Peer support will be provided by pharmacy colleagues within the PCN and across Leeds.

well-established and innovative PCN of 5 general practices with a long history
of working together and working with a range of local providers, including
across primary care, community services, social care and the voluntary sector,
to offer more personalised, coordinated health and social care to around 80,000
people.
The PCN works
hand-in-hand with general practice staff and employs a wide variety of roles
outside of the pharmacy team, namely; social and well being prescribers,
paramedics, occupational therapists, health and wellbeing coaches, nurse
associates, health care assistants and a phlebotomist. The PCN team work
closely with each other and meet monthly as part of a multi-disciplinary team
to discuss complex patients. There is a strong emphasis on learning and
development of PCN staff and as such the PCN have created a supportive
environment for staff to flourish.
