Job summary
We have an exciting, new opportunity for a Social and Wellbeing Prescriber to join our PCN.
Social and Wellbeing Prescribing empowers people to take control of their health and wellbeing through referral to dedicated workers, who give time, focus on what matters to them and take a holistic approach to an individuals health and wellbeing, connecting people to community groups and statutory services for practical and emotional support. Social and Wellbeing Prescribers also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.
The work is challenging but rewarding and you will need to be resourceful, creative and highly organised with the ability to respond to change and manage a varied workload and work independently. Main duties of the job
This role will be aligned to one of the 5 PCN practices.
Working hours will be flexible between 8.30am and 7.30pm. One or two days each week will include a late start/finish.
The post holder will support individuals to build independence and personal resilience, enabling self management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual need. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access.
Social and Wellbeing Prescribers will build and develop relationships with a wide range of services and activities in order to address social and health care needs, signposting and offering support to individuals to participate and benefit from community-based activity.
