As an ANP/ACP, you will
run your own clinics at the surgery with a mix of telephone and face to face
appointments. We have a great support network, both clinically and
administratively. You will be joining our current clinical team of
2 GP Partners, 1 Salaried GP, 1 ANP, 1 NP, 2 Practice Nurses and 1
HCA. We are a teaching and research practice. We are offering a
good salary (depending on experience) NHS pension, annual leave and bank
holidays (pro rata). We are part of the Fens and Brecks PCN.

Main duties of the job
The post
holder will be a caring, compassionate and committed experienced practitioner
who, acting within their professional boundaries, will provide care for the patient
from initial history taking, clinical assessment, diagnosis, treatment, and
evaluation of care. They will demonstrate safe, competent, and critical decision-making
and expert care for patients within the general practice. They will communicate
and work collaboratively with the multi-disciplinary practice team to meet the
needs of patients while supporting the delivery of policy and procedures.

Clinical Practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
