Job summary

The Clinical Pharmacist plays a vital role in supporting safe, effective, and evidence-based prescribing across the practice. They will conduct structured medication reviews, manage patients with long-term conditions, and provide expert advice on medicines to patients and colleagues

This is a patient-facing role that combines clinical work with wider responsibilities in medicines optimisation, prescribing safety, and service development. The pharmacist collaborates closely with GPs, nurses, and community pharmacy colleagues to improve outcomes, manage workload, and contribute to the smooth running of the practice.

The post-holder also supports CQC compliance, contributes to audits and quality improvement projects, and helps deliver national and local performance indicators such as QOF & IIF. This is a key role at the heart of the multidisciplinary team.

Main duties of the job

Structured Medication Reviews:Conduct reviews for patients with long-term conditions, polypharmacy, or high-risk medications to optimise treatment and improve outcomes.

Prescribing Support:Independently manage repeat prescriptions, respond to medication queries, and implement prescribing changes in line with safety alerts and local guidelines.

Chronic Disease Management:Support the care of patients with conditions such as hypertension, asthma, and diabetes, working within your clinical scope.

Medicines Reconciliation:Ensure safe transitions of care by reviewing and updating medications after hospital discharge or other care transfers.

Medicines Optimisation:Promote safe, evidence-based, and cost-effective prescribing, contribute to formulary compliance, and support shared care agreements.

Quality and Safety:Contribute to QOF, IIF and audit work; support CQC compliance; and assist with clinical governance and risk management.

Collaboration and Training:Work closely with GPs, nurses, and local pharmacy teams, and provide education and mentoring to practice staff on medicines-related issues. The role also involves mentoring foundation pharmacist staff members.

Drayton Medical Practice has been caring for the people of Market Drayton and the surrounding area since 1968.

We have an average list size of over 17,900 patients and offer a wide range of facilities and skills while maintaining the traditional values of family medicine.

In 2005 we moved to purpose built premises that offer scope for future expansion of both services and the practice team.

In addition to being a training practice for GP trainees, we also provide placements for student Doctors, Physician Assistants and Nurses. Trainees are well supported by all members of the practice team.

Other community services located within the premises and with whom we have good links include Physiotherapy, Dental, District Nurses, Midwives and Health Visitors.

We are a partner practice of Our Health Partnership, a GP partnership of 34 practices with 43 surgeries. 176 GP partners and circa 80 salaried GPs in Our Health Partnership serving around 300,000 patients in Birmingham, Sutton Coldfield, Wolverhampton, Solihull and Shropshire.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are a member practice of North Shropshire Primary Care Network (PCN), a network of GP practices covering Oswestry, Ellesmere, Whitchurch, Wem, Prees, Hodnet and Market Drayton.

The post-holder will play a key role in medicines optimisation, prescribing safety, long-term condition management, and clinical governance.
