Job summary

Newhall Surgery is looking for a dynamic and motivated Health & Wellbeing Coach. The successful applicant will act within their professional boundaries working alongside our Clinical Team. The role will be very much involved in the development of the practice, responding to and performing well with local and national initiatives, being involved with reporting, target setting and target achievement. They will work as part of a multi-disciplinary team to improve the quality of patient care.

Hours: 25 to 37.5 per week.

Closing date: 7 April 2024.

Main duties of the job

The post holder will use their health coaching skills to support individuals with low levels of motivation to develop their knowledge, skills and confidence to manage their own health and wellbeing and lead a more fulfilling life. This role will be an approach that considers the whole person in addressing existing issues and encourages proactive prevention of new and existing illnesses. There will also be involvement with patients who may need additional support and guidance through certain stages of their primary care journey.

The successful applicant with take an approach that is non-judgmental, based on effective communication and negotiation skills, which supports personal choice and positive risk taking that addresses potential consequences and ensures the individual understands the accountability of their own decisions.

This is a varied role and much of the work undertaken will be of a highly confidential and sensitive nature.

Newhall Surgery is a dynamic and forward-thinking Training Practice with an active Patient Participation Group, we are active on Facebook, liaise well within our local community and receive good reviews on NHS Choices.

We have a patient list size of 11,000. Our Clinical team consists of three GP Partners, four Salaried GPs, two GP Registrars, a Pharmacy Team and Nursing Team.

We also have a committed and professional Management Team, plus dedicated and patient focussed Administrative, Secretarial and Care Navigation team members.

We pride ourselves on the experience we give to GP registrars and medical students. We are high QOF achievers. SystmOne is our choice of clinical system.

We are a friendly Practice who believe that communication, team working, well-being and learning are vital to the success of our organisation. We also have two 'get togethers' per year to help support our internal communication processes and the feeling of 'family' within the organisation.
