Job summary

An opportunity for a Primary Care Practitioner to join our multi-disciplinary team and broaden our clinical skill-mix. This will be a varied role and the successful candidate will be a qualified nurse practitioner or specialist paramedic who will treat patients for Acute Illness over the telephone or face to face, at the surgery and in the patients own home as part of our Same Day Service (SDS). The Stennack Surgery has a Minor Injuries Unit, and youmay also be involved in treating injuries and emergencies (with the suitable qualifications).

  • Nurse Practitioner or Advanced Nurse Practitioner with Acute Illness Experience OR Specialist Paramedic with 2 years post registration experience of treating Acute Illness in Primary Care (or comparable experience) (Essential)
  • General Practice or Out of Hours Service Experience of treating patients for acute illness within Primary Care (or comparable) experience (Desirable)
  • Minor Injuries Qualification/Experience (Desirable but not essential)
  • Prescribing Qualification (Desirable but not essential)
  • Up to 37.5 hours per week (Applications for part-time hours welcome)
  • Salary £35,392- £57,349 depending upon experience and qualifications.

Main duties of the job

  • Diagnosing and treating patients presenting with minor illness.
  • Triage and treat patients wishing to see a health care professional for acute illness and minor injuries, making any necessary referrals to other members of the primary health care team.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
  • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary.

Responsible for attending to patients autonomously who wish to be seen by a health care professional normally same day; diagnosing and treating patients presenting with acute illness and minor injury; seeing and advising people in respect of their continuing medical and nursing needs. These patients will have booked appointments normally at 15-minute intervals. There is a requirement to see extra or emergency patients. The Primary Care Practitioner will need to prioritise and triage the needs of patients, accordingly, making any necessary referrals for investigations in the appropriate manner. A Duty Doctor is always available for advice and assistance. Patients with problems needing referral to secondary care will normally be discussed with the Duty Doctor before making such referral.

Job responsibilities:

Ensure the clinical computer system is kept up to date, with accurate details recorded and amended.

Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.

Attend and participate in practice meetings as required.

Restocking and maintenance of clinical areas and consulting rooms.

Supplies and equipment:

To assist with maintenance of equipment and stock relating to patient care as required

Training and personal development:

Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.

Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring PREP requirements are met.

If it is necessary to expand the role to include additional responsibilities, full training will be given.

Develop and maintain a continuous personal development plan.

Personal/Professional development:

The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include:

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

Liaison:

As well as the clinical team, there is a need to work closely with reception, admin, and management staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person.
