Job summary
The Primary Care Network (PCN) Lead
Manager will play a crucial role in the development of the PCN, supporting
the PCN Project Manager and managing services delivered through the PCNs.
The PCN Lead Manager will work closely with the PCN Clinical Directors,
PCN Project Manager and the PCN Practice Managers within the WEB area and,
where required, across the East Devon Health footprint.
PCNs are critical to the changing NHS and are the focal point of all
locally developed planning and service delivery. This role will be key to the success of the
PCNs enabling them to create clear plans for their own development and to initiate
new services.
Main duties of the job
Work with the PCN Clinical Director and Management
Team to:
- Ensure the PCN functions efficiently and
effectively
- Manage PCN Board meetings
- Facilitate members of the PCNs and, where
appropriate, key partner organisations to create and monitor PCN
development plans.
- Participate in PCN meetings facilitating discussion
to achieve consensus on areas where developments and/or sharing
resources would be beneficial
- Identify and develop alternative models of care
that will improve primary healthcare provision
- Work with the PCN practices to design new
services
Manage
PCN services:
- Support and line manage PCN Project Manager and additional roles staff including
responsibility for sub-contracted elements
- Ensure services work smoothly with member
practices
- Ensure PCN services meet their contract requirements and agreed performance metrics
- Ensure all new PCN staff members and
contractors receive appropriate induction and meet compliance requirements
- Provide all PCN staff members and, where
appropriate, contractors with appropriate support including mentoring and
appraisal
Work
with the Practice Managers to:
- Ensure the PCN support practices to deliver
services efficiently
Work
with member Practices to:
- Promote WEB PCN services, strategic
direction and brand ensuring appropriate levels of usage
of services and engagement
- Promote WEB PCN services to other organisations and individuals
About us
We are the Woodbury, Exmouth and Budleigh (WEB) Primary Care Network (PCN) a group of 6 East Devon GP practices working together to deliver integrated primary care services across our combined population of over 52,000 patients.
Our 6 WEB practices have a long history of collaboration and support and as a PCN we believe in building strong relationships between our practices and with our local services and our population.
Our practices are now working more meaningfully together and we have established a range of services with the introduction of practice Pharmacists, Physiotherapists, a Frailty Support team and a Mental Health & Wellbeing team.
The PCN is supported by a small and enthusiastic management team who are passionate about providing services that benefit practices to manage their workload and free up the time needed to support complex patients and the population to receive regular and specialist care in their local neighbourhood to help foster a sense of community.

DUTIES AND RESPONSIBILITIES OF THE POST

plans for new services to ensure they are well planned, communicated and
delivered on time

appropriate levels of usage of services and engagement. Liaising with partners, salaried GPs, nurses,
secretaries and other staff
- Promote WEB PCN services to other organisations and individuals within
the PCN geography

- To
be sufficiently confident in skills and knowledge to manage own workload
efficiently and effectively on a daily basis.
- Follow
PCN practice policies and protocols.
- Maintain
and update own training relevant to the post and take an active part in the development
review of own work suggesting areas for learning and development in the coming
year.
- Be
focused on solutions and a can-do proactive approach.
- Attend
regular meetings with Line Manager to agree priorities and overall approach.
