An exciting opportunity has arisen
for a practice nurse role at Station Lane Medical Centre due to a planned
retirement. This is open to-
Experienced practice nurses,
New or recently qualified nurses
Nurses who have not worked in general practice
before.
We have the opportunity to enrol a new to
General practice nurse into the vocational training scheme which will allow
for a year of supported teaching and learning alongside working in the
practice. This will start November 2025.
We are looking to recruit a
Practice Nurse to join our busy and friendly team here at Station Lane Medical
Centre. We would welcome applicants from community nursing or secondary care
settings. We offer excellent training opportunities and the candidate will be
able to join the VTS training scheme for the first 12 months of employment if
they are new to primary care or with less than 12 months experience in primary
care.
Working in accordance with the NMC
Code of Conduct, the post-holder will provide assessment, treatment, screening,
health education services and advice to patients.
The post-holder will work with
reference to practice agreed protocols and guidelines, providing nursing
treatments to patients both independently and in participation with nurse
colleagues and general practitioners.
The post-holder will have the
required level of training and competence and will work within patient group
directions where these are available.
Main duties of the job
Provide general nursing care, including wound
management, immunisations, and chronic disease management.
Conduct health assessments and screenings.
Support patients with long-term conditions,
providing education and self-management advice.
Participate in health promotion activities and
initiatives.
Work collaboratively with GPs, healthcare
assistants, and other practice staff.
Maintain accurate and up-to-date patient
records.

- Identifying significant abnormalities
- Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Wound Care and management
- Assessing routine wounds including trauma
- Assessing [and prescribing] to support good wound management in complex situations
Patient health checks
- Identifying significant abnormalities
- Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
- Working with patients to develop a management plan where health problems or potential health problems are identified
Risk assessment
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Health screening
- Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)
- Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
- Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health
- Administering injections and providing guidance in accordance with guidelines
- Identifying problems
- Provision of specialist and evidence-based support to the team.
Immunisation (adult and child)
- Administering appropriate immunisation autonomously
