Job summary

The Care Home Care Co-ordinator acts as the primary point of contact between the GP practice and associated care homes, ensuring effective communication, coordination, and continuity of care for residents. The role involves managing a dedicated care home email inbox and telephone line, liaising with care home managers and carers, and navigating patients to the most appropriate clinician using healthcare navigation skills.

Key responsibilities include organising referrals, arranging monthly proxy medication orders, registering and deducting residents, coordinating vaccinations, and ensuring annual care plans are completed and accurately recorded on EMIS. The post holder is responsible for organising and minuting multidisciplinary team (MDT) meetings and supporting the delivery of high-quality patient care.

The role requires excellent communication, organisation, confidentiality, and IT skills, alongside flexibility, reliability, and patience. Additional duties include maintaining a clean and tidy workstation, responding to general enquiries, facilitating appointment bookings and telephone consultations, and undertaking further training as required. Main duties of the job

The main duties of the Care Home Care Co-ordinator include acting as the primary liaison between the GP practice and care homes, managing dedicated email and telephone communication, coordinating patient care and referrals, arranging medication orders, organising vaccinations and care plans, booking appointments, supporting MDT meetings, maintaining accurate patient records on EMIS, and ensuring efficient navigation of residents to the appropriate healthcare professionals.
