Main duties of the job

Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Job description

Job responsibilities

The Clinical Pharmacist will:

  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
  • Be a qualified prescriber or be happy to undertake training to become a prescriber.
  • Ensure full responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, especially the elderly, people in care homes, those with multiple long term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).
  • Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network and help in tackling inequalities.
  • Undertake audits as required by the Practice and implement appropriate measures to effectively act on the results.
  • Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services.
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
  • Support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
  • Develop relationships and work closely with other pharmacists across networks and the wider health system.
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.
  • Participate in regular supervision sessions and appraisals with your line manager. Attend training and development activities as identified and participate in meetings as required.
  • Maintain administration systems and workspace used in a clean and tidy condition.
  • Maintain a good working knowledge of Health and Safety procedures and fire precautions, and operate the correct procedures and participate in policy development and data collection where appropriate.
  • Work flexibly to meet the needs of clients.
  • To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.
  • Act on own professional judgement on a daily basis with regard to making recommendations in respect of medicines management
  • To work as a clinical pharmacist within a practice/(s) and, typically within defined local protocols that have been approved by GPs. To be responsible and accountable for own actions, working independently within professional and defined organisational boundaries.
  • Plan and prioritise own workload.
  • Initiate and manage meetings with prescribers and other practice staff and make records as appropriate.
  • Undertake any other duties that are commensurate with the post and within the post holders clinical competence.
