Job Description Practice Nurse (with Long-Term Conditions Management)

1. Job Purpose

To provide high-quality, evidence-based nursing care to the practice population, with a particular focus on theprevention, early detection and management of long-term conditions (LTCs)such as diabetes, COPD, asthma, cardiovascular disease and hypertension.

The postholder will:

• Deliver nurse-led clinics for LTCs in line with national and local guidelines (e.g. NICE, local ICB pathways).
• Support achievement of QOF, IIF and other quality/improvement indicators.
• Work as part of the multidisciplinary team (MDT) within the practice and PCN to improve outcomes and reduce health inequalities.
• Contribute to proactive, population-based care, including recall systems and structured reviews.

2. Key Duties and Responsibilities

2.1 Clinical Care Long-Term Conditions

• Undertakestructured annual and interim reviewsfor patients with LTCs (e.g. diabetes, COPD, asthma, CHD, heart failure, hypertension, CKD), in line with practice protocols and national guidance.
• Perform and interpret relevant clinical measurements, for example:
• Blood pressure, pulse, BMI, peak flow/spirometry (if trained and accredited), foot checks, urinalysis, blood glucose monitoring, etc.
• Use templates and recall systems to ensuresystematic, proactive managementof LTC registers.
• Provideindividualised self-management support and education, including lifestyle advice (smoking cessation, weight management, physical activity, diet, alcohol).
• Identify patients at risk of deterioration or complications andescalate to a GP or appropriate clinicianas per practice protocols.
• Contribute tomedication monitoring(e.g. BP checks, inhaler technique, insulin administration teaching, adherence support) under the direction of a prescriber, without independently initiating or altering medication regimes.
• Support delivery ofanticipatory and proactive carefor frail and high-risk patients as part of PCN/ICS pathways.

2.2 General Practice Nursing

• Provide a full range of treatment room and practice nursing services, which may include:
• Wound care and dressings
• Cervical cytology (if trained and accredited)
• Childhood and adult immunisations and vaccinations
• Contraception and sexual health support (within competence and local pathways)
• Travel health advice and vaccinations (where provided by the practice)
• ECGs, venepuncture, and other procedures as required and trained
• Participate ininfection prevention and controlmeasures and audits.
• Providehealth promotion and disease preventionactivities, including NHS Health Checks where appropriate.

2.3 Multidisciplinary Working and PCN Responsibilities

• Work collaboratively with GPs, other practice nurses, healthcare support workers, clinical pharmacists, social prescribers, health and wellbeing coaches, and other ARRS roles.
• Contribute to PCN-wide initiatives on:
• CVD prevention and diagnosis
• Proactive care for people with frailty and multiple LTCs
• Health inequalities work (e.g. CORE20PLUS5 priorities)
• Attend and contribute to MDT meetings, case discussions and clinical meetings as required.

2.4 Quality, Governance and Safety

• Work withinNMC Code of Professional Standardsand maintain professional registration and revalidation.
• Adhere to practice and PCN policies, protocols and clinical guidelines.
• Maintain accurate, contemporaneous and complete clinical records on the practice clinical system.
• Participate inclinical audit, significant event reviews, and quality improvement projectsrelated to LTC management and general practice nursing.
• Recognise and act onsafeguarding concerns(children and adults) in line with practice policy and statutory requirements.
• Understand and work within the boundaries of own competence, seeking advice and escalating concerns appropriately.

2.5 Education, Training and Development

• Maintain and develop clinical skills and knowledge in line with thePrimary Care and General Practice Nursing Career and Core Capabilities Framework.
• Participate in annual appraisal and agree aPersonal Development Plan (PDP).
• Attend mandatory and role-specific training as required by the practice/PCN and NMC.
• Support the induction, supervision and development ofHCAs, student nurses, nursing associates, and other learnerswhere appropriate.
• Engage with PCN and ICS training opportunities relevant to LTC management and population health.

2.6 Administrative and Organisational

• Contribute to the development and review ofpractice protocols and care pathwaysfor LTCs and general practice nursing.
• Assist with recall systems, registers and searches to support QOF, IIF and other contractual requirements.
• Participate in service planning and development discussions, particularly around LTC clinics and proactive care models.

Experience

Essential

• Post-registration nursing experience (e.g. minimum 1 year)
• Experience of working in primary care or community settings
• Experience of working within a multidisciplinary team

Desirable

• Experience in long-term conditions management (e.g. diabetes, COPD, asthma, hypertension, CHD)
• Experience of using clinical IT systems (e.g. EMIS, SystmOne, Vision)
• Experience of participating in clinical audit, QI projects or QOF work

Knowledge and skills

Essential

• Understanding of the role of general practice nursing within the NHS and PCN context
• Knowledge of evidence-based management of common LTCs (within nursing scope)
• Ability to perform and interpret routine clinical observations and tests (e.g. BP, BMI, peak flow, urinalysis, blood glucose)
• Competence in wound care, dressings and basic treatment room procedures
• Knowledge of safeguarding (children and adults) and information governance
• Good IT skills , including use of electronic patient records, templates and recall systems
• Awareness of health inequalities and the need for personalised, culturally sensitive care

Desirable

• Understanding of QOF and its relevance to LTC management

Qualifications

Essential

• Registered Nurse (Adult) with the Nursing and Midwifery Council (NMC)
• Evidence of recent CPD relevant to primary care / LTC management

Desirable

• Post-registration qualification(s) in one or more LTC areas (e.g. diabetes, respiratory, cardiovascular disease)
• Cervical cytology training and current accreditation
• Immunisation and vaccination training (adult and/or childhood)
• Mentorship / practice assessor qualification or equivalent

Other requirements

Essential

• Willingness to undertake further training relevant to the role and service needs
• Ability to travel between sites within the PCN if required
• Up-to-date occupational health clearance and immunisations as required
