Job responsibilities
1.
Develop and implement the digital
and IT transformation plan with the intention of ensuring that the PCN and
its member practices consistently utilise the latest technology where
appropriate to improve patients services, maximise clinical efficiency,
enhance internal communications and support improved information management.
This will include reviewing and improve the PCNs digital maturity and embedding
digital processes as a foundation for staff working practices and to support
delivery of contract requirements.
2.
Project management lead on project
management across the PCN to deliver workstreams. Manage
the service transformation projects for the PCN. This includes the range of
projects that cover clinical and non-clinical areas.
3.
Build relationships and facilitate
collaboration between practices and the wider system to support the delivery
of care to patients.

Strategy and Leadership provide
operational leadership

Identify opportunities for improvements to
support practices and PCNs manage workload

Use data, and improve data quality,
to:
understand demand, capacity and activity
Drive improvements in patient experiences of access and staff
experience at work.

through OD programmes
facilitate clinically led innovation and the effective adoption
of improvement initiatives, including integrated working at neighbourhood
and/or place level to improve access to services for patients.
Measure impact of interventions (inc project reporting)
1.
Effective
facilitation to drive forward the networks objectives and goals through
meetings and workshops as required, create an inclusive environment,
effectively setting guidelines and ensuring everyone has a voice to achieve
the desired outcome in a timely manner.
2.
Ensure
that projects are effectively managed throughout their duration and
communicate with and support member practices where appropriate. This work
will include reporting on specific milestones and outcomes for projects
3.
Identify
innovative schemes and projects. Oversee the roll out and delivery of new
services working alongside the project manager.
4.
The
role will require good analytical skills and numeracy, in-depth knowledge of
the NHS from both commissioning and providing perspectives, including an
understanding of the process to implement and monitor the delivery within pathways
of care.

To
support the network in delivering high quality, patient-centred services.
6.
You
will be expected to develop and establish strong collaborative relationships
with partners and stakeholders and will be expected to contribute to
performance monitoring of locality services.

Support
a shared identity and branding within the network.
8.
Ensure
there are robust systems and processes in place for the network to operate as
efficiently and effectively as possible.
9.
Work
with the PCN Management to ensure the network complies with all relevant and
legislative responsibilities relating to GDPR, Health & Safety, and Human
Resources.

Develop and maintain an in-depth
understanding of the PCN digital infrastructure and business intelligence
tools
11.
Effective
use of business intelligence systems / data sets to support the development
of the PCN and integrated care. To interpret and analyse information which
can be highly complex, sensitive and/or contentious

To
analyse complex facts and information and interpret them for comparative
purposes and for improving service delivery and/or project outcomes.
13.
Facilitate
conversations to bring about changes across the system, based on good
practice, patient experience or variation in performance, where appropriate,
in a way that is supportive and non-judgemental.
14.
Build
excellent relationships with partner organisations to both support the
development of the network and the wider integrated care system.
15.
Use
knowledge of local and national strategies and frameworks to further develop
the work of the networks, seeking opportunities to gain additional funding to
enhance the scope and pace of development.
Drive the planning and prioritisation of digital changes that will
improve the delivery of care and improve patient and staff experience in the
practices within the PCN.
Ensure all change is focussed on improving services for patients.
Build relationships across the PCN to develop consensus for
improvements and to ensure business and clinical staff are fully engaged
throughout the process of selection, implementation, and deployment of
change.
Work alongside practices to understand current challenges and identify
solutions. This includes providing hands-on support with implementing and
sustaining agreed changes.
Support practices and staff to effectively use relevant technology
systems and products to enable improvement.
Advise on innovative opportunities and support practices to maximise
benefits from innovation, through the sharing of learning and best practice.
Identify and support with training and development, with opportunities
for upskilling staff to become digital champions.
Develop and manage an overall digital transformation plan for the PCN
that also includes identifying and managing risks, considering the potential
impacts on the wider organisation, and determining resource requirements.
Manage and monitor the performance of the PCN ensuring the delivery of
high-quality services and achievement of targets.
Review existing datasets to analyse the utilisation of digital tools
at a practice and PCN level
Use practice and PCN data from clinical systems to aid with
Population Health Insights
Improving Access and appointment planning
Improving Efficiency through reviewing demand, activity and
utilisation of appointments and how
patients use consume the services
Workforce planning
Use data, improve data quality, to understand
demand, capacity and activity and drive improvements in patient experience of
access, operational efficiency including better matching capacity to need and
staff experience at work
Ability to interpret qualitative and quantitative information from a
range of sources and present it concisely to stakeholders to increase their understanding
of operational challenges.
Liaise with system suppliers to optimise their development of tools to
support transformation

with practice staff, private providers, the local primary care team,
neighbouring PCNs and other key partners to enable good working relationships

non-technical and engaging way to a range of stakeholders

Lead or participate in relevant
working groups, project groups, service meetings and other forums across the
PCN to agree changes, share learning, discuss challenges, and resolves issues
