Care Coordinators play a key role within a PCN to support patients to access the care and services they need. As part of the PCN team, Care Coordinators support proactive healthcare by contacting patients and helping them to engage with important preventative services such as cancer screening programmes, immunisations and chronic disease monitoring. Providing coordination and navigation of care and support across health and care services. Care Coordinators also support patients to become more digitally enabled.

The role focuses on proactive patient outreach and coordination of care, helping patients to navigate services across General Practice, community health services and the voluntary sector.

This role primarily involves contacting patients to support proactive care initiatives and follow-up activity. The role is not clinical and does not involve managing routine GP appointment requests or undertaking general reception duties.

Care Coordinators help patients access preventative care, overcome barriers to healthcare and support the Primary Care Network to improve health outcomes across our population.

The role requires someone who enjoys speaking with people, is highly organised and can manage a structured workload in a busy Primary Care environment.

  • GSCE grade A to C in English and Maths

Experience

  • Experience working with patients, the public or service users to support uptake of services or interventions

Desirable

  • Experience of working directly in a Care Coordinator role, healthcare, social care or community services
  • Experience of working within multidisciplinary teams
  • Experience of supporting people, their families and carers in a related role

Knowledge and skills

  • Friendly, professional and approachable manner
  • Ability to actively listen, empathise and provide personalised support
  • Confident speaking to patients by telephone on a regular basis Commitment to equality, diversity and reducing health inequalities
