Job summary

The purpose of this role is to provide a comprehensive pharmacy service to patients of North Oxfordshire Rural Alliance Primary Care Network (PCN).

This role is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

While we are happy to consider flexible working, PCNs need to provide patients with enhanced access and you may therefore on occasion be required to work weekday evenings 18:30 20:00 and/or Saturdays 09:00 17:00.

Main duties of the job

The post holder is a GPhC registered pharmacy technician. They will act as part of a multi-professional NHS team in general practice and outreach settings across North Oxfordshire providing clinical pharmacy services to our registered patients. The post holder will help patients to get the best from their medicines through timely medicines reconciliation after discharge and on other transfers of care, medication reviews, identification of patients suitable for electronic repeat dispensing, promotion of online ordering, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence. The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially in relation to repeat prescribing systems and processes. The post holder must enrol upon or apply to be exempted from the CPPE 15 month primary care pharmacy education pathway.

In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. This will involve assisting the PCN in achieving national service specification requirements, NICE implementation and utilisation of medicines optimisation initiatives.

The successful candidate will be based in a local cluster of General Practices as part of North Oxfordshire Rural Alliance (NORA) Primary Care Network (PCN). NORA PCN Practices are at Bloxham & Hook Norton Surgery, Chipping Norton Health Centre, Cropredy Surgery, Deddington Health Centre, Wychwood Surgery.

Undertake medication reviews

Day to day handling of prescription queries within area of own competence

Delivery of training and development to practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within General Practice

Support the role of the clinical pharmacists in NORA PCN specifically reviews of housebound or post-discharge patients in their own homes or in care homes

Use and maintain information reporting systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings, and patient outcomes

Participate in practice meetings, patient participation groups, and other meetings to improve engagement of the role of pharmacy technicians and to promote issues relevant to prescribing and medicines optimisation

Liaise with other pharmacy stakeholders e.g. Medicines Optimisation team, community pharmacy, and hospital pharmacy teams

Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct

Utilise GP practice clinical systems to conduct prescribing audits e.g. EMIS Web

Undertake, participate and share the outcomes of medication audits and quality improvement projects to facilitate improvement and changes in practice

Contribute to the review and development of repeat prescribing protocols and standard operating procedures to improve the efficiency and effectiveness of repeat prescribing

Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result

Carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change

Establish and maintain effective working relationships with GP practices, ICB staff and other healthcare professionals/stakeholders across the interface

Provide clinical and professional support to GPs and members of the primary healthcare team

Provide direct support and advice to GP practices and health professionals to promote and implement high quality prescribing to the benefit of patients

Promote cost-effective, safe, evidence based prescribing in accordance with national guidance and the local medicines optimisation strategy

Qualifications

Essential

  • Registration with the General Pharmaceutical Council
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences

Desirable

  • Membership of the Association of Pharmacy Technicians UK (APTUK)
  • Membership of the Primary Care Pharmacy Association (PCPA)
  • Postgraduate qualifications of a relevant nature. e.g. FdSc in Medicines Management/Pharmacy Services; BTEC Therapeutics

Experience

  • Minimum 2 years post qualification experience
  • Experience of working as a qualified, registered pharmacy technician in primary care, hospital or community pharmacy

Desirable

  • Experience and awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice
  • Experience of partnership / collaborative working and of building relationships across a variety of organisations
