Job summary

An exciting opportunity has arisen within the Foundry Primary Care Network to support patients within their own homes and communities. Receiving referrals directly from clinicians, you will work within a small, supportive team to help patients with mental health conditions to achieve independence and remain socially resilient. Working across 8 GP Practices with a culturally diverse demographic, your role will be varied, flexible, challenging and rewarding.

Main duties of the job

You will offer emotional and practical support to patients who have a wide range of clinical and social needs. You will assist and coach individuals to ensure they are making healthy and positive choices, that they have access to the right funds, appropriate housing and they are connected to their communities.

The nature of the role will vary for each individual on your caseload, and you will tailor your approach to suit their needs. You will liaise directly with clinicians and provide feedback both verbally, and on clinical IT systems.

Job responsibilities

As an Emotional Well-being Worker within the Foundry Primary Care Network and contribute to the delivery of excellent patient care

To provide one to one support, both emotionally and practically, to a cohort of identified patients with complex mental health and social needs.

To communicate effectively with patients, colleagues, and stakeholders both within the Network and externally.

To contribute to team and service development and planning

To attend and contribute to meeting and multi-disciplinary meetings that support the Networks vision.

To work build and maintain good professional relationships both within the Network and externally and effectively collaborate with other agencies and partners.

To be committed to ongoing professional development, including safeguarding and mental capacity training.

To work proactively within the team and other teams to ensure integrated multidisciplinary working is successful and embedded throughout the Network.

  • Commitment of supporting individuals with complex mental, physical, emotional, and practical needs, and their families and carers.
  • Ability to build trust with patients to support them to set realistic life goals and achieve positive outcomes, whilst respecting own professional boundaries.
  • Ability to advocate and negotiate with or on behalf of the individuals and identify resources to support them, including but not exhaustive of housing, benefits, community connectedness, social interests etc.
  • Ability to work within diverse communities, and tailor own approach to respect religious and cultural needs.
  • Ability to communicate effectively with a wide range of audience both verbally and written.
  • Ability to effectively work on own initiative and actively seek out solutions and information that will benefit patients and the wider Network.
  • Ability to manage own caseload of patients with complex clinical and social needs.
