Job summary

JOB PURPOSE:

To develop the role of nurse practitioner within the practice.

To share workload and responsibilities with GPs enabling the practice to enhance the services provided.

To deliver a minor ailments service to patients using safe clinical decision-making, assessment and diagnostic skills.

To work collaboratively with the GPs, nursing team and Practice Manager to meet the needs of patients and support the delivery of policy and procedures.

Chronic disease management.

To jointly work on the development of practice clinical and managerial protocols alongside the Practice Nurse and GPs.

To actively contribute to the development of new waysof working.

Main duties of the job

MAIN DUTIES AND RESPONSIBILITIES

CLINICAL PRACTICE

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients.

Clinically examine and assess patient needs and plan clinical care accordingly.

Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary

Prescribe and review medication in accordance with practice and national protocols.

COMMUNICATION

Utilise sensitive communication styles to ensure patients are fully informed and consent to treatment

Maintain effective communication with the practice team

DELIVERING A QUALITY SERVICE

Recognise and work within own competence and professional code of conduct as regulated by the NMC/RCN

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Deliver care according to NSF, NICE guidelines and evidence-based care.

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.

Support and participate in shared learning across the practice and wider organisation.

Llanilar Health Centre is based in the beautiful village of Llanilar, only 6 miles from the University town of Aberystwyth, close to the beautiful Cardigan Bay coastline and the Cambrian Mountains.

We are looking for a Advanced Nurse Practitioner to join our team to provide outstanding care to our diverse population of 3400 patients.

We are a high achieving practice who offer a friendly work environment with a GP Partner, a Salaried GP, an Advanced Nurse Practitioner, Nurse Practitioner, Pharmacy Technician, HCA, Phlebotomist and which benefits from an onsite dispensary. Our reception and dispensary team are highly efficient, friendly and helpful.

Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines

Ensure appropriate supervision of safe storage, rotation and disposal of vaccines and drugs. Oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements where appropriate.

MANAGING INFORMATION

Use technology as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.

Collate, analyse and present clinical data and information to the team.

CRITICAL REQUIREMENTS

Ability to work with minimal supervision after initial training period.

An understanding, acceptance and adherence to the need for strict confidentiality of both patient records and business activities of the partnership.

Excellent communication skills.
