General Practice Nurse

Job summary

The ideal candidate would be a qualified Practice Nurse with some Long Term and Chronic Disease Management experience and ideally specialising in COPD/Asthma (but not essential as training can be provided)

Main duties of the job

  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients
  • Identify, manage and support patients as risk of developing long-term conditions
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide wound care (ulcer /Doppler etc.)
  • Provide specialist clinics such as contraception etc.
  • Process and interpret pathology and other test results
  • Provide chronic disease clinics
  • Maintain accurate clinical records
  • Maintain chronic disease registers
  • Implementation of immunisation programme
  • Chaperone
  • Support the team in dealing with clinical emergencies
  • Implement vaccination programmes for adults and children
  • Contribute to practice targets (QOF etc.)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
