Job summary
Nene Valley Hodgson Medical Practice have an exciting opportunity for a highly motivated, experienced Clinical Pharmacist to join our existing dynamic pharmacy team; currently consisting of 2 x Pharmacists, 1 x Pharmacy Technician. The successful candidate will work across 2 GP Practices (Nene Valley & Hodgson) as part of the larger PCN, helping to develop medicines optimisation services. We are looking for an individual who is enthusiastic and passionate about providing high quality care for our patient population.
This is an innovative and developing role, taking advantage of the NHS Network Contract DES for Additional Roles Reimbursement Scheme (ARRS).
As part of a larger multi-disciplinary team, the role will provide a mix of face-to-face, patient-centred consultations and telephone support to patients and external agencies. You will be expected to use your clinical skills to carry out a variety of tasks including: medication reviews, medicines reconciliation, and reducing inappropriate prescribing in risk-stratified patients.
Main duties of the job
Practice as an
independent prescriber working within scope of practice (or work towards this
qualification)
Carry out
face-to-face and telephone consultations with patients with minor illnesses
To provide
medication review services with patients in the practice and during domiciliary
visits to the local nursing home where indicated
To provide advice
and support for patients with self-limiting conditions Encourage
cost-effective prescribing within the practice
To reconcile
medicines following patients discharge from hospital or clinic appointments
To provide
subject matter expertise on medication monitoring,
Carry out
targeted medication reviews for high-risk patients
To reauthorise
medication for repeat prescribing within scope of practice
Identify patients
suitable for repeat prescribing, liaising with the relevant clinicians as required
Review and
implement safe prescribing systems for vulnerable patients, improving
compliance
Support
clinicians with the management of patients suffering from drug and alcohol
dependencies, this is considered a specialist area so support and direction from
specialist services (Aspire for example) may need to be sought and followed.
Actively signpost
patients to the correct healthcare professional
Assist with QOF
targets
Partake in audit
as directed by the audit lead
Provide education
on medicines for the clinical team
Review the latest
guidance ensuring the practice conforms to NICE, CQC etc.

Nene Valley Hodgson Medical Practice look after 19,400 patients across two GP Surgery sites, we have a really strong and diverse clinical and non clinical team, including Partners, Salaried Doctors, Pharmacists and Pharmacy Technician, ANP's Practice Nurses, HCA's, Social Prescribing Link workers and Care Co-ordinators, First Contact Physio and we are active in our PCN. We are a fully active training practice and host GP Registrars, Foundation Year 2 doctors and Medical Students.

The post holder will work as
