Job summary

A fantastic opportunity has arisen, due to retirement, for a 5 session salaried GP to join our friendly, enthusiastic and forward-looking team at Burngreave Surgery.

List size around 8000 patients. We serve an inner city, ethnically diverse population with many patients born outside the UK including a sizeable proportion of refugees. Our patients are resourceful, but the health needs are high. At present we also have a smaller branch surgery at Herries Road which is open on 4 mornings a week. We are committed to delivering high quality patient care in a challenging environment. Over the last few years, we have strived to have community languages on reception.

We are entering an exciting time of change. We will be moving to a new publicly owned building in the spring. Currently being built, it is sited on Spital Street and is built to high environmental standards. The practice will be the largest tenant occupying the first floor whilst the ground floor will have much needed shared bookable space for the network and local community. At the time of moving our branch surgery at Herries Road will close but we are expecting an overall expansion of the list around the time of moving. Our training capacity is currently limited by space but we are intending to train GP registrars once settled in new build.

Main duties of the job

Currently, we are specifically looking for someone to do a role which is split between core clinical general practice and care homes. At present there is one session of care home work available but due to changes anticipated early next year, the proportion of care home work would increase to two or more sessions so an interest and confidence , or developing confidence, in frailty and end of life care is vital.

We aim to support and nurture the skills and development of our staff. We are a welcoming team and try and look after each other with good working relationships across administrative and clinical staff. We know that everyone on the team has a vital part to play. Regular refreshments provided during clinic. We have a sociable staff room with a supply of fresh fruit and there is chat and company at lunch on most days. Being very central the practice is easy to reach on bike foot or by public transport.

Our core clinical team currently consists of 4 GP partners 4 salaried GPs 1 FY2 resident doctor 2 Advanced Nurse Practitioners 2 Practice nurses 2 Health Care Assistants 2 Pharmacists a social prescriber and a visiting Physio.

We also host student nurses on placement and medical students with an interest in working in a Deep End area or in the Greener Practice initiative.

Job responsibilities

JOB PURPOSE

A key member of the clinical teams, the GP will manage a case load, dealing with a wide range of presenting health care needs in a GP setting. The post holder will work with clinical colleagues to ensure the highest standard of care is provided for all registered and temporary patients. The post holder will also contribute to planning and decision making as a part of the wider practice team.

MAIN DUTIES AND RESPONSIBILITIES

Clinical Responsibilities

  • In general the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care including;
  • Making professionally autonomous decisions in relation to presenting problems, whether self referred or referred internally within the practice
  • Receive patients with undifferentiated and undiagnosed problems and make assessments of their health care needs
  • Consult with patients in the surgery, at home visits and via telephone as agreed internally between the practice GPs
  • Undertakes triage calls, visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion as required.
  • Screen patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current practice disease management protocols, develops care plans for health
  • Provide counselling and health education as necessary
  • Admit or discharge patients to and from caseload and refer them to other care providers internally and externally as appropriate and in line with practice protocols.
  • Comply with all relevant clinical Governance and Practice policies and protocols.
  • Assist in the establishment of appropriate systems to manage common chronic medical conditions.
  • Complete clinically related administrative and non clinical duties required for the delivery of the service.
  • Record clear and contemporaneous consultation notes to agreed standards.
  • Demonstrate commitment to lifelong learning, and audit and effectiveness to ensure evidence based best practice.
  • Provide active input to the wider Primary healthcare team in the process of health needs research/ base line data collection/clinical audit.
  • Compile and issue computer generated acute and repeat prescriptions, prescribing in accordance with practice/Sheffield CCG prescribing formulary whenever this is clinically appropriate.
  • Network with other agencies (both statutory and voluntary) and develop appropriate links to further the work of the practice and the neighbourhood
  • Meet professional learning and CPD requirements.
  • Develop effective relationships with clinical supervisor/mentors and other colleagues.
  • Contribute to the Practice achievement of QOF and other performance targets
  • It would be desirable if the applicant had working knowledge of SystemOne
  • A keen interest in managing health inequalities would be advantageous

PRACTICE RESPONSIBILITIES

  • Contribute to the evaluation/ audit and clinical standard setting with colleagues.
  • Record data in patient records systems promptly and accurately and to agreed standards.
  • Ensure appropriate use of read codes.
  • Participate in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits.
  • Participate in the auditing of practice activity as required.
  • Contribute to the strategic management and development of the practice.
  • Provide reports to both NHS and non-NHS agencies as required.
  • Responsible for health and safety of themselves and others and for reporting and actively co-operating in Alliance and Practice Health and Safety Policies and procedures.
  • Process and action incoming patient and hospital correspondence.
  • Attend in house meetings and other meeting as deemed necessary.
  • Ensure that the practice complaints system is adhered to at all times.
  • Awareness and compliance with all relevant guidelines e.g. data protection, confidentiality, and health and safety.
