Job summary

Part time Practice Nurse with Special Interest in Diabetes required for a large Practice in Kidderminster with 15400 patients. Hours negotiable

Main duties of the job

We are looking for an enthusiastic individual to join our practice nursing team. You will be responsible for the routine, evidence-based care of patients with Diabetes, including annual reviews incorporating local prescribing guidelines as well as following up patients with more complex diabetic needs including insulin transfers.

We work from a modern purpose built centre in Kidderminster.

We are a high achieving, GMS, CQC registered Good Practice.

We have a Lead GP for diabetes and a very supportive clinical team.

We also have a dispensing branch site in Cookley.

Job responsibilities

Weekly Hours: Part Time hours negotiable. May need to work during extended access hours

Reporting to:Partners,Practice Manager & Lead Nurse

Essential Qualifications:NMC Registration

Role of the department:Assist GPs in the care of practice patients to include treatment, preventative care, screening & patient education.

Main Duties:

Clinical

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with chronic diseases or complex needs with the support of our GPs.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.

Diagnose and manage both acute and chronic conditions, integrating both pharmacological and non-pharmacological based treatment methods into a management plan.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national, local & practice-based protocols.

Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed and over-the-counter medication regimens, side-effects and interactions.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Carry out those activities involved when assessing patients including but not limited to diagnostic tests such as blood tests, peak flow readings, spirometry and ECGs.

Recognise the significance of the observations made and use these to develop a plan of care, with the support of the Clinical Nurse Manager.

Treatment of wounds, dressing and management of leg ulcers, removing sutures, routine injections, childhood immunisations & ear syringing where required.

Advising and giving immunisations and vaccinations as appropriate for patients travelling abroad.

Taking and despatching laboratory specimens including blood, urine, swabs and cervical smears.

Recognise and work within own competence and professional codes of conduct as regulated by the NMC.

Administrative

Prioritise, organise and manage workload in a manner that promotes quality.

Produce accurate, contemporaneous and complete records of all patient contacts, consistent with legislation and practice procedures.

Support and participate in shared learning across the practice.

Liaison with and referral to other agencies.

Assist the management and admin teams in assessing and implementing new enhanced services and QOF requirements.
