Clinical:
Undertake nursing care and tasks:
Patient health checks Cardiovascular Disease
Blood pressure monitoring including ambulatory monitoring
ECG recording
24 ABPM desirable
Urinalysis
Weighing
Phlebotomy
Administration of intramuscular injections
Administration of flu & pneumonia injections at the surgery only
Chaperoning duties
INR testing and monitoring using INR star software - desirable
Dementia screening
Mechanical falls screening
Simple wound management
Removal of sutures/staples
Weight management and diet advice
Routine diabetic checks in line with the 9 key processes including foot checks and care
Planning - desirable
Care planning for complex patients in order to pro-actively manage and co-ordinate patient
care so as to avoid hospital admissions
Simple medication monitoring to ensure compliance
Disposal of practice clinical waste
Assist with patients in their homes as required with relation to nursing care/needs or care planning - desirable
Undertaking basic post discharge monitoring within a set protocol to ensure that the discharge has been safe for the patient and ensure appropriate follow up - desirable
Record activities on agreed templates using SystmOne electronic records system.
Report any abnormal findings to on call general practitioner for follow up and investigation within an appropriate time frame depending on the level of need.
Liaise with outside agencies, such as health, social and voluntary agencies, and other surgeries in our health network on behalf of patients.
Collecting, capturing and collating required data on health and wellbeing in planned and opportunistic patient encounters and settings to support the on-going work of the surgery.
Assist the surgery team during a medical emergency within the surgery.
Be trained and have annual updates on CPR and anaphylaxis.
Assist in surgical/medical equipment and vaccine stock control/re-stocking and rotation.
Vaccine/cold chain storage, monitoring & recording.
Processing and management of laboratory samples requested by GP/nurses ensure samples are labelled and bagged ready for collection with necessary forms.
Care & maintenance of equipment including annual/periodic calibration.
Monitoring, ordering and close stock control of all medical supplies including annual audit of stock.
Non Clinical
Be part of the practice team, attending and participating in practice meetings.
Be responsible for maintaining and continuing professional development in line with the needs of the surgery and nursing team.
Participate in annual appraisal with line manager.
Contribute, when requested, to participate in teaching sessions within the surgery setting.
Good time keeping.

Infection Control
Infection control is everyones responsibility. The post holder will adhere to and promote within the team the local trusts Infection Prevention and Control Policies and make every effort to maintain high standards of infection control at all times thereby reduce the burden of healthcare associated infections including MRSA. In particular all staff should have the following key responsibilities:
Staff must wash their hands or use alcohol hand rub before commencing clinic and between each patient contact.
Staff have a duty attend infection control training provided by the local NHS trust.
Post holder should take lead in screening surgery staff should an infection be detected or reported within the clinical team.
