Job summary
Eltham Medical Practice is seeking an experienced and motivated Practice Nurse to join our friendly and supportive primary care team.
This is an excellent opportunity for a nurse who is passionate about delivering high-quality, patient-centred care in a dynamic general practice setting.

Main duties of the job
Deliver a full range of nursing services to our patient population, including immunisations, cervical cytology, wound care, long-term condition management, and health promotion.
Provide evidence-based care in accordance with practice protocols and national guidance.
Support chronic disease management clinics (e.g. diabetes, asthma, COPD, hypertension).
Undertake and interpret investigations such as ECGs, blood pressure monitoring, and spirometry (if trained).
Maintain accurate clinical records using EMIS Web.
Contribute to QOF, IIF, and other quality targets within the practice.
Participate in multidisciplinary team meetings, audits, and service development.
