Job summary

Eltham Medical Practice is seeking an experienced and motivated Practice Nurse to join our friendly and supportive primary care team. This is an excellent opportunity for a nurse who is passionate about delivering high-quality, patient-centred care in a dynamic general practice setting.

Main duties of the job

  • Deliver a full range of nursing services to our patient population, including immunisations, cervical cytology, wound care, long-term condition management, and health promotion.

  • Provide evidence-based care in accordance with practice protocols and national guidance.

  • Support chronic disease management clinics (e.g. diabetes, asthma, COPD, hypertension).

  • Undertake and interpret investigations such as ECGs, blood pressure monitoring, and spirometry (if trained).

  • Maintain accurate clinical records using EMIS Web.

  • Contribute to QOF, IIF, and other quality targets within the practice.

  • Participate in multidisciplinary team meetings, audits, and service development.

    Job title: Practice Nurse Responsible to: Nurse Lead / Partners Hours: Part-time (negotiable for the right candidate) Salary:£22-25 per hour, dependent on experience

    Key responsibilities:

    • Provide nursing care to patients of all ages, including wound management, cervical screening, immunisations, and health checks.

    • Support the management of long-term conditions through review clinics and patient education.

    • Administer and monitor vaccinations and injections in line with national guidelines.

    • Provide lifestyle advice and promote self-care and health improvement.

    • Participate in infection control, clinical audits, and cold chain maintenance.

    • Work collaboratively with the multidisciplinary team to deliver coordinated care.

    • Maintain professional standards and comply with NMC revalidation requirements.

    Desirable

    • Travel health experience
