Job summary

Eltham Medical Practice is looking for an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly, forward-thinking primary care team. This is an exciting opportunity to work within a large, well-established practice that values innovation, teamwork, and high-quality patient care.

The successful candidate will play a key role in delivering autonomous clinical care, supporting our multidisciplinary team, and contributing to ongoing service development as we continue to expand our patient list.

Main duties of the job

  • Provide high-quality clinical assessment, diagnosis, treatment, and management of patients presenting with a wide range of undifferentiated and undiagnosed conditions.

  • Undertake acute illness / disease assessment and management, including prescribing and reviewing medications within scope of practice.

  • Support urgent on-the-day demand alongside the duty team.

  • Deliver proactive, patient-centred care for long-term conditions in collaboration with the wider clinical team.

  • Request and interpret investigations, make referrals, and liaise with other health and social care professionals as appropriate.

  • Contribute to clinical governance, quality improvement, and the development of care pathways.

    Hours: Full or part-time (negotiable) and Locum opportunities available

    Salary:£60,000-66,000 base range dependent on experience (negotiable). Hourly Locum rates available £40 per hour

    Key responsibilities:

    • Undertake patient consultations in surgery, no home visiting expectations

    • Work independently to assess, diagnose, plan, implement, and evaluate treatment plans within scope.

    • Prescribe and review medication in accordance with local and national guidelines.

    • Identify, manage, and refer patients appropriately, ensuring safe and effective clinical decision-making.

    • Maintain accurate and contemporaneous patient records.

    • Participate in clinical meetings and contribute to the ongoing development of practice services.

    • Minimum of 2 years post-qualification experience as an ANP or equivalent autonomous role
    • Proven ability to assess and manage acutely unwell and chronically ill patients in a primary care setting
    • Experience in practices using triage, prescribing, and working within a multidisciplinary team

    Desirable

    • Experience supervising students or junior clinicians
    • Experience with EMIS Web clinical system
    • Interest or expertise in specific long-term condition management (e.g. asthma, frailty)

    Qualifications

    Essential

    • Registered Nurse with valid NMC registration
    • MSc or PG Diploma in Advanced Clinical Practice (or equivalent experience)
    • Independent Nurse Prescriber (V300) qualification
    • Evidence of ongoing professional development

    Desirable

    • Experience supervising students or junior clinicians
    • Experience with EMIS Web clinical system
    • Interest or expertise in specific long-term condition management (e.g. asthma, frailty)
